Retatrutide: what the research measured

Retatrutide — reta, in the way most people search for it — has a short list of measured outcomes and a long list of claimed ones, and the gap between them is what this page is for. Every figure below came out of a registered trial, and four outcomes account for nearly all of them.

Body weight carries the most evidence. In a 2023 phase 2 trial, 338 adults took a weekly injection or placebo for 48 weeks, and an independent review of 26 trials later put the top figure at up to 22.1%. Blood sugar fell across two diabetes trials, liver fat fell by up to 82.4% in a substudy of 98 participants, and a 2024 review of this family records effects on blood pressure, cholesterol, blood glucose, and insulin.

Read what that list leaves out. These trials ran 40 to 48 weeks in adults chosen by body size and blood sugar, and they measured scales, scans and laboratory readings rather than feelings. Sleep apnea and knee pain are registered endpoints in studies that published nothing, and appearance, energy, mood and athletic performance were not endpoints in any of the trials behind this page.

None of it is approved. Retatrutide is still counted among premarket agents for long-term weight management, it has no brand name because a brand belongs to something a regulator has cleared, and every benefit on record was measured in material supplied inside a trial.

Is retatrutide good for weight loss?

On body weight the answer was measured rather than argued, in a randomised trial carrying a placebo group. In the 2023 phase 2 obesity trial, participants received subcutaneous retatrutide or placebo once weekly for 48 weeks, and the primary end point was the percentage change in body weight from baseline to 24 weeks.1 An independent reading of the field landed in a similar place. A 2025 systematic review of 26 randomised trials reports that retatrutide (12 mg once weekly) produced greater weight loss of up to 22.1% (CI, 19.3% to 24.9%) after 48 weeks.7 Hold the design alongside the number, because the design is what the number means. Those figures are group averages over a fixed window, in adults enrolled against written criteria, and the bracketed interval is the reviewers' own estimate of how far the figure might move. An average measured against placebo is a finding about a group rather than a forecast for one person.

Can retatrutide help with belly fat?

Belly fat is not an endpoint in any of the retatrutide trials behind this page, and the nearest figure is a relationship noticed inside the liver substudy. In that study, liver fat reductions were significantly related to changes in body weight, abdominal fat and metabolic measures associated with improved insulin sensitivity and lipid metabolism in the participants examined.6 Read that carefully, because the grammar is doing the work. Abdominal fat appears there as something liver fat moved along with, not as a target anybody set out to shift. A relationship inside one substudy is a weaker thing than a measured result. The group it came from narrows it further. Everyone in that substudy had been selected for carrying at least a tenth of their liver as fat, so the relationship belongs to those ninety-eight people.6 Fat around the middle and fat inside an organ are different questions, and only the second one has published numbers behind it.

What does retatrutide do for type 2 diabetes?

It moved a blood-sugar average, and two trials measured that same quantity at different stages of development. HbA1c is roughly the share of haemoglobin carrying sugar, read as an average over about three months. In the 2023 phase 2 diabetes trial, the change at 24 weeks ran from -0·43% (SE 0·20; -4·68 mmol/mol [2·15]) for the 0·5 mg group at the bottom to -2·02% (0·11; -22·07 mmol/mol [1·21]) for the 12 mg escalation group at the top.2 That trial also ran an active comparator arm, and the figure recorded there was -1·41% (0·12; -15·40 mmol/mol [1·29]) for the 1·5 mg dulaglutide group.2 The investigators' own conclusion is the careful version. In people with type 2 diabetes, retatrutide showed clinically meaningful improvements in glycaemic control and robust reductions in bodyweight, in their words.2 Clinically meaningful is a judgement made by the people running the trial rather than by a reader, and it describes an average across a group recruited on written criteria.

What else changed in the trials apart from weight?

Three further measures have figures attached, and each reaches us through a review rather than a trial built to test it. A 2024 review of this family records that these compounds induce weight loss in adults with overweight and obesity, alongside beneficial impacts on blood pressure, cholesterol, blood glucose, and insulin.4 A 2025 review devoted to retatrutide adds two more to the list. Across the phase 1 and phase 2 work it describes dose-dependent weight loss, reductions in Glycated Hemoglobin (HbA1c) levels, and improvements in liver steatosis and diabetic kidney disease, steatosis meaning fat stored in the liver.10 Diabetic kidney disease is the entry nobody repeats. It appears in that review as a single clause with no figure beside it and no trial named, so what exists is a summarised improvement rather than a measured one a reader can weigh. A finding condensed into a review and a result printed in a trial report are different things.

What is retatrutide being tested for beyond obesity?

Two complications of obesity, in a programme designed to report on each separately. A 2026 paper describing those studies says the programme evaluates safety and efficacy concurrently across obesity and two related complications-obstructive sleep apnea (OSA) and knee osteoarthritis (OA).11 The endpoints are specific, and both are unusual on this site. For sleep apnea the measure is change in Apnea-Hypopnea Index, a count of breathing interruptions per hour of sleep.11 For the knee it includes change in the Western Ontario and McMaster Universities Osteoarthritis Index pain subscale score, which is a questionnaire a patient fills in rather than a scan. One of the four studies is a stand-alone OA trial, and a separate one recruits a population with CVD, meaning cardiovascular disease.11 None of them has published a result, so each of these is a registered question rather than a reported benefit.

What does taking retatrutide feel like?

No trial among the research behind this page asked anybody. These studies weighed people, drew blood and scanned livers, and not one of those readings describes an experience from the inside. The closest published proxy is the adverse-event record, which catalogues what participants noticed rather than what they gained. In the 2023 phase 2 obesity trial the commonest events were gastrointestinal, they were dose-related, and they were mostly mild to moderate in severity.1 That is a record of discomfort rather than of how appetite or energy felt day to day. What the hormone literature describes is the signal underneath. A 2024 review of gut hormones groups GLP-1 and GIP among the anorexigenic hormones, meaning hormones that turn appetite down, and sets ghrelin against them as an orexigenic one that turns it up.3 Appetite inferred from a hormone and hunger as somebody actually experiences it are different questions, and only the first has been studied in this compound.

Does retatrutide work for everyone?

Not to the same degree, and the phase 2 obesity trial published the spread as well as the mean. At 48 weeks, a weight reduction of 5% or more, 10% or more, and 15% or more had occurred in 92%, 75%, and 60%, respectively, of the participants who received 4 mg of retatrutide.1 The arm above it reached 100%, 91%, and 75% of those who received 8 mg on those same three thresholds.1 So at the middle assignment two participants in five did not reach a tenth of their starting weight, while at the next one up three in four passed a fifteenth. Responder thresholds are worth understanding on their own terms. A threshold counts how many people crossed a line, whereas an average describes how far the whole group moved. A trial can report a large mean change while many participants sit below it.

Does retatrutide work for menopausal women?

Nothing among the research behind this page reports an outcome split by menopausal status, and no trial used it as an entry condition. Recruitment ran on body size and blood sugar instead. Age is the only published proxy, and it is a loose one. A 2025 systematic review of 26 randomised trials describes participants with a mean age, 34 to 57 years across the studies it pooled.7 The 2026 phase 3 trial recorded that baseline mean age was 48·8 years (SD 12·1), which places a substantial share of its women on either side of the usual transition.12 So women before and after menopause sat inside those ranges and were never counted apart. That is an unmeasured question rather than a null result, and a subgroup nobody analysed cannot be said to have responded the same way, or differently.

Is retatrutide used for cutting or by athletes?

No trial among the research behind this page has enrolled athletes, or measured strength and body composition for the sake of appearance. The outcomes on record are body weight, blood sugar and liver fat, in adults recruited for obesity or diabetes rather than for training. The measured finding that bears on cutting runs the other way. A 2024 review of this family reports that these compounds induce ∼15-24% weight loss in adults with overweight and obesity, incretin being the shared name for them.4 The same review records that they also cause rapid and significant loss of lean mass (∼10% or ∼6 kg), comparable to a decade or more of aging.4 Their proposal is the part worth carrying away: they propose that tailored resistance exercise training be recommended as an adjunct to incretin therapy to optimize changes in body composition by preserving lean mass while achieving fat loss.4 That pairing has not been tested alongside retatrutide in any trial behind this page, so it is advice about a class rather than a result about this compound.

Has retatrutide been studied in dogs or other animals?

In laboratory animals, yes, and in pets, no. A 2025 review of the whole record states that animal studies demonstrate retatrutide's ability to delay gastric emptying, reduce food intake, and promote weight loss, with superior efficacy compared to other incretin-based therapies.10 That work sits underneath the human trials rather than beside them. It was run to establish that a molecule reaching three receptors behaved as its designers expected, and the same review reports that Phase I and II clinical trials corroborate these findings in people.10 Gastric emptying there means the speed at which a stomach passes food along. Nothing among the research behind this page describes giving retatrutide to a dog, a cat or any other companion animal, at any amount, for any purpose. That is an absence of published work rather than a finding about what would happen, and the two are easily confused.

What is another name for retatrutide?

Its laboratory code is LY3437943, and that string is what the trial literature pairs with the generic name. The 2023 phase 2 obesity trial opens by stating that Retatrutide (LY3437943) is an agonist of the glucose-dependent insulinotropic polypeptide, glucagon-like peptide 1, and glucagon receptors.1 There is no brand name, and the reason is structural. A brand belongs to something a regulator has cleared for sale, and a 2025 systematic review still sorts the field into 3 commercially available agents [liraglutide, semaglutide, and tirzepatide] and nine premarket ones, with retatrutide among the second set.7 The practical use of the code is searching. A study listed only under LY3437943 is a retatrutide study, and the registry entries, the trial reports and the reviews all use one label or the other. Anything sold under a name that turns up in neither sits outside that record rather than inside it.

Does retatrutide protect the heart?

No retatrutide trial among the research behind this page reports a heart outcome. What exists for the wider class belongs to other molecules. A 2024 review records that the development of GLP-1 receptor agonists was followed by data establishing the cardiorenal benefits of GLP-1RA in select patient populations.5 Cardiorenal there means the heart and the kidneys together, and select means some groups of patients and not all of them. Notice which words carry that sentence. It is about the older one-receptor compounds, in chosen groups. Retatrutide turns up later in the same review as a molecule that is still being tested. A phase 3 study of retatrutide in people with obesity and heart disease is listed as completed, and no result from it has been published.13 A 2025 systematic review still records that data on mortality and obesity-related complications, such as cardio-renal-metabolic events, are needed.8 A benefit borrowed from a class and a result measured in one compound are different questions.

What does retatrutide do to appetite and the stomach?

This is the part most often described and least often measured. A 2024 review of gut hormones explains the arrangement it works inside: various gut hormones interact with the brain through delicate communication, thereby influencing appetite and subsequent changes in body weight.3 Retatrutide reaches two of those gut signals and adds a third hormone on top. A 2025 review describes the design as potent activation of GLP-1, GIP, and glucagon receptors, leading to significant weight reduction, improved glycemic control, and favorable metabolic outcomes in the trial literature it surveys.10 What no study among the research behind this page has published is an appetite measurement in a person given this compound. The slowing of the stomach was recorded in animals, the weight change in people, and the step between the two is an inference rather than a reading. The same review of gut hormones calls for integrated and largescale long-term studies in this field, which is its authors' way of saying the chain is not yet closed.3

Why did one molecule need three receptors?

Because the field advanced by adding receptors, each addition tested as a compound of its own. A 2025 review of the pipeline lays the sequence out, describing results obtained with GLP-1 mono-agonists, GLP-1/GIP dual agonists, GLP-1/glucagon co-agonists, and the triple agonist retatrutide (GIP/GLP-1/glucagon), which have shown beneficial effect both on body weight and steatotic liver disease.9 That is four groups in a single sentence, and retatrutide is the only occupant of the fourth. A 2024 review adds that investigational molecules such as maritide block the GIP and activate the GLP-1 receptor, which shows these receptors can be pushed in either direction rather than merely switched on.5 What is missing is a trial separating retatrutide's three actions. No study among the research behind this page gave the glucagon action on its own and measured what it contributes. The case for the third receptor rests on the design and on the size of the numbers rather than on an experiment that isolated it.

What we don’t know

The gaps in the evidence matter as much as the findings.

  1. 01How much of the weight change is fat. No retatrutide trial among the research behind this page reports body composition, and the lean-mass figure quoted everywhere comes from a review of the wider family.
  2. 02What any of it feels like. Appetite, energy and mood were not endpoints in these trials, so the only record of subjective experience is the adverse-event list.
  3. 03Whether the glucagon action adds anything on its own. No study among the research behind this page gave that action separately and measured the difference.
  4. 04What happens in sleep apnea or knee osteoarthritis. Both are registered endpoints in studies that have published nothing.
  5. 05Whether there is a cardiovascular benefit. The cardiorenal evidence in this field belongs to other molecules, and the retatrutide trial in people with heart disease has reported nothing.
  6. 06Whether results differ before and after menopause, or between men and women. Both sexes were enrolled in numbers and no analysis among the research behind this page separates them.
  7. 07What diabetic kidney disease improvement amounted to. It appears in one review we cite as a clause with no figure and no trial named beside it.
