Retatrutide is a research compound that switches on three gut and pancreatic hormone receptors at once, and the short version is that the human evidence is unusually strong while the approval is not there.

The weight figures come from randomised trials rather than from animals. In a 2023 Phase 2 study, 338 adults with obesity took a weekly injection or placebo for 48 weeks, and mean body weight fell 24.2% in the 12 mg group against 2.1% on placebo. A 2025 systematic review of 26 trials and 15,491 participants, searching the field independently, put the figure at up to 22.1%, and separate Phase 2 work in fatty liver disease reported liver fat down 82.4% at 24 weeks.

That is further into people than almost anything else on this site. Phase 2 is finished in obesity, diabetes and liver disease, a 40-week Phase 3 trial randomised 537 adults with type 2 diabetes and has reported, and four more Phase 3 studies in over 5,800 participants are running. What none of them has answered is what happens over years, or after stopping.

It is not approved by the FDA or any other regulator for any indication, and the agency's enforcement record already carries a recall of a compounded retatrutide injection for lack of assurance of sterility.

What exactly is retatrutide?

Retatrutide is a research compound given as a weekly injection under the skin. It is studied in obesity and type 2 diabetes, and searched for under the short name reta. A 2026 trial report calls it a GIP, GLP-1, and glucagon triple hormone receptor agonist, under clinical development for type 2 diabetes, obesity, and related complications4. Those three names belong to receptors for hormones the gut and pancreas release around a meal. An agonist is a molecule that switches a receptor on. Reaching all three at once is the design idea, and it is what sets retatrutide apart from the single-target and dual-target compounds built before it. The teams running the late-stage trials describe it as a novel synthetic molecule5, made in a laboratory rather than taken from anything living. One company makes it, and makes it for one trial programme.

Is reta a peptide?

Yes. The 2023 diabetes trial calls it a single peptide with agonist activity at three receptors at once.2 The label is accurate and almost useless on its own, because the word peptide covers an enormous range of evidence. BPC-157 (body protection compound 157) is a peptide too, and nearly all of what is known about it was measured in rats. Retatrutide sits inside a funded late-stage programme with thousands of randomised participants. What stands behind the two is not the same kind of thing at all, and the shared word is the least useful fact about either. What it imitates is a signal the body already runs. A 2024 review of gut hormones notes that GLP-1, GIP and several others are known as anorexigenic hormones, meaning hormones that turn appetite down.11 The same review places retatrutide among the compounds that are effective in weight loss and improving various metabolic parameters associated with obesity.11

Do retatrutide peptides actually work?

On body weight, yes. The 2023 Phase 2 obesity trial enrolled 338 adults, and assigned them to weekly retatrutide or placebo once weekly for 48 weeks.1 At 48 weeks, the least-squares mean percentage change in the retatrutide groups was -8.7% in the 1-mg group, -17.1% in the combined 4-mg group, -22.8% in the combined 8-mg group, and -24.2% in the 12-mg group, as compared with -2.1% in the placebo group.1 That design is double-blind, placebo-controlled and dose-ordered, about as clean as a result in this field gets. An independent reading of the literature arrived at the same place. A 2025 systematic review went through 26 RCTs comprising 15 491 participants, and found that retatrutide (12 mg once weekly) produced greater weight loss of up to 22.1% (CI, 19.3% to 24.9%) after 48 weeks.6 Glucose responded as well, the 2023 diabetes trial concluding that in people with type 2 diabetes, retatrutide showed clinically meaningful improvements in glycaemic control and robust reductions in bodyweight2. The limitation belongs beside the result. That same review states that no head-to-head RCTs were available, and that heterogeneity prevented meta-analysis6, so every league table ranking these compounds was assembled from separate trials in separate populations. What is retatrutide peptide used for?

In the published record it is studied for four conditions, and every one of them is metabolic. The late-stage programme covers obesity and two related complications-obstructive sleep apnea (OSA) and knee osteoarthritis (OA).5 The fourth is type 2 diabetes, where a 2026 Phase 3 trial tested it alone, in adults whose diabetes is inadequately controlled by diet and exercise alone4. Liver fat is the finding people rarely hear about. A 2024 Phase 2a study randomised 98 participants with fatty liver disease.3 At 24 weeks, normal LF (<5%) was achieved by 27% (1 mg), 52% (4 mg), 79% (8 mg), 86% (12 mg) and 0% (placebo) of participants3, where LF abbreviates liver fat. Nobody receiving placebo reached a normal reading, and most people at the top amount did. What the record does not contain is any study of retatrutide for appearance, recovery, sleep quality or athletic performance. Those applications are unexamined rather than weakly supported, which is a different thing entirely. How quickly does retatrutide work?

The published clock begins at 24 weeks, where the obesity trial took its first scheduled measurement. The least-squares mean percentage change in body weight at 24 weeks in the retatrutide groups was -17.5% in the 12-mg group, as compared with -1.6% in the placebo group.1 Set that beside the 48-week figures and the trajectory becomes legible: at the highest amount, roughly three quarters of the eventual weight change had accumulated by the halfway point. Liver fat followed a comparable schedule, the mean relative change from baseline in LF at 24 weeks reaching -82.4% (12 mg) and +0.3% (placebo)3. What no trial among the research behind this page reports is week one, week four or week eight, because the amount is escalated gradually and the primary end point was the percentage change in body weight from baseline to 24 weeks1. So the question people actually ask, which is when they will notice something, has no measured answer. These trials were constructed to detect a difference at six months rather than to describe a first month, and those are different questions.

What does reta do to your body?

Three effects are documented, and the third is not the one most people anticipate. The first concerns appetite. Animal studies demonstrate retatrutide's ability to delay gastric emptying, reduce food intake, and promote weight loss12, gastric emptying being how quickly the stomach passes food along. Those readings were taken in animals; the human trials recorded the downstream result rather than the mechanism itself. The second is glucose. Across Phase I and II clinical trials, reviewers report dose-dependent weight loss, reductions in Glycated Hemoglobin (HbA1c) levels, and improvements in liver steatosis and diabetic kidney disease12, where HbA1c is a three-month average of blood sugar and steatosis is fat in the liver. The third deserves sitting with. A 2024 review of this class reports that they also cause rapid and significant loss of lean mass, which it puts at roughly a tenth of body weight and calls comparable to a decade or more of aging.8 Weight on a scale is fat and muscle together, so a 24% fall in body weight is not a 24% fall in fat.

Why am I not losing weight on retatrutide?

The trial data confirm that variation between individuals is genuine, and they show how wide it is. At 48 weeks, a weight reduction of 5% or more, 10% or more, and 15% or more had occurred in 100%, 93%, and 83% of those who received 12 mg; and 27%, 9%, and 2% of those who received placebo.1 Read the first of those columns closely. Everybody on the highest amount had lost at least a twentieth of their body weight, and seventeen participants in every hundred still had not reached a fifteenth. That is the spread inside a monitored trial, with supplied material and a fixed schedule, which makes it the floor on variation rather than the ceiling. Two features of those trials are easily left out. The amount was escalated in steps across months, so somebody early in that climb is not receiving the amount that produced the headline number. And every Phase 3 study administers the compound in conjunction with healthy diet and physical activity5, meaning the published results describe a combination rather than an injection alone. Has retatrutide been tested in people?

Repeatedly, at every stage, and the chain is worth seeing in order. Phase 1 came first, and the 2023 diabetes trial refers back to it: retatrutide showed clinically meaningful glucose-lowering and bodyweight-lowering efficacy in a phase 1 study2. Phase 2 then ran in two populations. The diabetes arm drew its 281 people from 42 research and health-care centres in the USA, and these phase 2 data also informed dose selection for the phase 3 programme.2 Phase 3 has now reported once. In a 40-week trial at 48 sites in the USA, Mexico, and India, 930 participants were screened and 537 were randomly assigned4, and the report notes that the trial is registered with ClinicalTrials.gov, NCT06354660, and is completed4. Four more are under way. The late-stage programme runs four Phase 3, multicenter, randomized, double-blind studies assessing weekly subcutaneous retatrutide compared to placebo, in over 5800 participants.5 The public registry lists A Study of Retatrutide (LY3437943) in Participants Who Have Obesity or Overweight as COMPLETED14.

What happens when you stop taking retatrutide?

No study among the research behind this page has followed anybody after stopping retatrutide. The Phase 2 trials ended at 48 weeks and the Phase 3 diabetes trial ran 40 weeks, so nobody in these sources has been observed coming off it. What exists instead is an expectation about the whole class. A 2024 review discusses retaining lean mass during incretin therapy as a way to blunt body weight (and fat) regain on cessation of weight loss pharmacotherapy8, which makes regain the assumed baseline rather than the surprise. A 2025 review of the field is blunter: the outstanding question is maintenance of the weight loss, possibly pharmacological treatment needs to be life-long.13 So the honest answer has two halves. What is expected, from how related compounds behave, is that weight returns. What has been measured in this compound is nothing, because a 2025 review records that long-term follow-up data on the safety and efficacy of weight maintenance with novel obesity pharmacotherapies are still needed7. An expectation borrowed from a drug class and a measurement in one compound are different questions.

Is retatrutide FDA approved?

No. Not for obesity, not for diabetes, not for anything, and not in any other country either. A 2025 systematic review split the field into 3 commercially available agents [liraglutide, semaglutide, and tirzepatide] and 9 premarket agents for long-term weight management6. Retatrutide is in the second group. A 2025 review of the pipeline says the same, describing a compound that has progressed to phase 3 trials as obesity treatments10. That is a stage of testing, not a decision. The gap has a consequence worth knowing. Because no approved retatrutide exists, every vial outside a trial was made by someone working outside that system, and the FDA record shows what can go wrong. It lists Retatrutide for Injection, 60mg / 10mL vial, all presentations, GenoGenix, LLC, recalled for Lack of Assurance of Sterility.16 Read that reason closely: it is about how one batch was made, rather than about the molecule. When will retatrutide be approved by the FDA?

No source among the research behind this page gives a date, and nobody outside the company and the agency is placed to. What can be said is where the programme stands. Its own authors call the four late-stage studies registrational clinical trials, meaning studies designed to support an application.5 Designed to support an application is not the same as granted, and registrational trials do miss their endpoints. Alongside them the registry carries a direct comparison, Effect of Retatrutide Compared With Semaglutide in Adult Participants With Type 2 Diabetes, listed as ACTIVE_NOT_RECRUITING15. The more useful signal is what reviewers say is still absent. A 2025 systematic review of this pipeline states that data on mortality and obesity-related complications, such as cardio-renal-metabolic events, are needed.7 Weight on a scale and years of life are different endpoints, and regulators have been asking this whole class for the second one.

What are the pros and cons of retatrutide peptide?

Taking the record as it stands, rather than as either side describes it. In its favour, the evidence is the kind this site almost never gets to report: randomised, placebo-controlled, dose-ordered, in hundreds of people, repeated across obesity, diabetes and liver disease. A 2025 review of the pipeline writes that combinations of entero-pancreatic hormones approach the WL achieved with bariatric surgery, where WL means weight loss.10 Against it, three things. Reviewers of this pipeline say that data on mortality and obesity-related complications, such as cardio-renal-metabolic events, are needed.7 Nothing has been measured after stopping. And a 2025 review raises considerations regarding cost and the quality of weight loss beyond BMI reduction, which is the muscle question again in formal language12. None of that resolves into a verdict, and it is not meant to. It is the shape of what is known, which is the part a reader cannot easily assemble alone.

Is retatrutide safe?

What is documented is a pattern; what is missing is time. In the Phase 2 obesity trial, the most common adverse events in the retatrutide groups were gastrointestinal. These events were dose-related, were mostly mild to moderate in severity, and were partially mitigated with a lower starting dose (2 mg vs. 4 mg)1. The same trial recorded that dose-dependent increases in heart rate peaked at 24 weeks and declined thereafter.1 Across the wider class, a 2025 systematic review found adverse events frequent among trial participants and predominantly in the gut, most commonly nausea, vomiting, diarrhea, and constipation6. That is a short-term picture, collected in monitored trials of 40 to 48 weeks, in people screened before they began, and it is not a description of years. How does retatrutide work?

Three switches, one molecule. A 2024 review notes that retatrutide and survodutide enable simultaneous activation of the glucagon and GLP-1 receptors.9 A third receptor, GIP, is added on top. The 2023 obesity trial names all three in full, as an agonist of the glucose-dependent insulinotropic polypeptide, glucagon-like peptide 1, and glucagon receptors.1 The first two are gut signals that fire after a meal. In animal work, compounds of this kind delay gastric emptying, reduce food intake, and promote weight loss.12 The glucagon receptor is the odd one out. Glucagon normally tells the body to release stored energy rather than to store more. A 2025 review of the animal and clinical work assembles the chain: Retatrutide's unique molecular structure enables potent activation of GLP-1, GIP, and glucagon receptors, leading to significant weight reduction, improved glycemic control, and favorable metabolic outcomes.12 What no trial among the research behind this page has done is separate the three. No study we could find has given the glucagon arm alone to people and measured what it adds. So the idea that the third receptor explains the bigger numbers is an inference from the design, rather than a measured finding.

The gaps in the evidence matter as much as the findings.
