Follistatin side effects and safety data

Follistatin has almost no safety record in people, because the research behind this page records only one human administration, and that was a gene carried into muscle by a virus. What is documented about harm comes from animals and from the follistatin the body makes on its own.

The sharpest signals sit in two places. In 72 rats given the protein by an implanted pump, muscles came out smaller in one group and weaker in another, and in 4,060 Swedish adults followed for 19 years, higher natural follistatin went with a higher risk of type 2 diabetes.

Beyond those, the protein reaches into fertility hormones, artery biology and cancer, where one set of mouse models found it drove resistance to immune-based cancer drugs while other mouse work used it to shrink tumours. None of that was measured in a person given follistatin, and the animal studies reporting no adverse effects are describing their delivery particles.

Follistatin is not an approved medicine in any record among these sources. A vial sold under the name names no isoform, carries no purity data that we could find, and has no human dosing study behind it.

Is it safe to take follistatin?

The honest answer starts with what was actually taken, because the evidence does not describe a vial of follistatin peptide going into a person. Among the research behind this page, the one human administration was a gene: a 2018 review describes intra-muscular injections of an isoform of follistatin (FS344) by AAV1, given alongside exercise in a small group of patients with inclusion body myositis.1 AAV1 is a modified virus used to carry a gene into muscle cells, so those patients were given instructions for making the protein rather than the protein itself. The review reports the outcome in terms of physical capacity, which improved in the patients who exercised more, and its summary gives no count of adverse events for the gene therapy.1 Its broad safety line, that all studies report safety of exercise in all types and stages of myositis, is a statement about the exercise and not about the gene.1

So the question splits in two. For a gene delivered by virus, there is one small patient study with no harm figures in its published summary. For a protein injected from a vial, there is no human record among these sources, and the closest evidence is a rat study in which the muscle results went the wrong way. An absence of reported harm in work that barely dosed anyone does not establish that the protein is harmless.

What harms were recorded where follistatin was actually delivered?

Four animal studies delivered follistatin in some form, and each reported something about tolerance. The most direct is a 2022 experiment in which 72 Sprague-Dawley rats with injured or sham-operated nerves received the FS-288 protein from a pump under the skin.2 In those rats the protein left muscles smaller after three months of denervation, and it reduced force in the sham group, whose nerve had been exposed in surgery but never cut.2 That is an unwanted effect on the very tissue the protein is bought for, even if it is not toxicity in the usual sense.

The others are gentler, and narrower. A 2024 study of follistatin mRNA in mice with head and neck cancer says the therapy also demonstrated a favorable safety profile.3 A 2026 lung tumour study in mice reports its particles preserving muscle and fat without adverse effects.4 The 2024 gene-delivery paper says its particles showed low immunogenicity, meaning they provoked little immune reaction, in mouse and non-human primate models.5 Each of those reassurances belongs to a delivery system tested in animals, rather than to a protein drawn from a vial into a syringe.

Does follistatin raise blood sugar?

This is the one harm question with real human numbers behind it, and they concern the follistatin the body already makes. A 2021 study opens by describing follistatin as a liver hormone that is elevated in patients with type 2 diabetes and promotes hyperglycemia, or high blood sugar, in mice.6 Followed forward, each step up in plasma follistatin carried an adjusted hazard ratio for type 2 diabetes of 1.24 across 19 years in 4,060 Swedish adults, and 1.31 across four years in 883 Finnish adults.6 A hazard ratio of 1.24 means the risk ran roughly a quarter higher for each standard step up in follistatin, after the authors adjusted for other factors.

In a third group of 210 German patients, high circulating follistatin went together with insulin resistance in fat tissue, which is fat that has stopped responding properly to insulin's signal.6 All of that is association with the body's own levels, measured against later diagnoses rather than against an injection. It does not establish that a dose from outside raises blood sugar in a person. It is, however, the clearest reason among these sources to want that question asked before anyone assumes the answer.

Is follistatin hard on the liver or kidneys?

The liver question has some human data, though it runs in a direction readers may not expect, and the kidney question has none for follistatin at all. In a 2016 study of eight men with cirrhosis and eight healthy men, the patients showed an impaired capacity to release follistatin when hormones were infused.7 The same patients carried significantly less lean mass in their limbs than the healthy men.7 That is a damaged liver making less follistatin, which is the opposite of follistatin damaging a liver. The 2021 diabetes study adds a second link, since in its German group high circulating follistatin went with non-alcoholic fatty liver disease, meaning fat built up inside liver cells.6

For the kidneys, the only work among these sources concerns a different protein: a 2025 study found that follistatin-like 1 rose in human kidney tubule cells exposed to a bacterial toxin.8 Silencing that protein in those cells cut their rate of cell death and raised cell viability.8 Follistatin-like 1 is a separate gene with a separate job, so that result says nothing about follistatin and kidneys. We could find no kidney measurement in any animal or person given follistatin among the research behind this page.

What else does follistatin block besides myostatin?

A great deal, which is why the safety questions reach well past muscle. A 1998 review describes a protein able to bind and neutralise the actions of many members of the Transforming Growth Factor-beta family of proteins, a large family of growth signals of which myostatin is only one.9 The name itself comes from fertility, since the same review says follistatin was first implicated in the regulation of follicle-stimulating hormone secretion in the pituitary.9

Activin is another of its targets, and the 2024 cancer study calls activin A a pivotal driver of metastasis and cachexia, the wasting seen in advanced illness.3 The same 1998 review notes the protein is much more broadly distributed than first thought and may also play a significant role during embryogenesis, the development of an embryo.9 A signal blocker that circulates reaches every tissue that uses those signals, not just the muscle a buyer has in mind. The next three answers follow those tissues one at a time: reproduction, cancer, and the arteries.

Could follistatin affect fertility or reproductive hormones?

These studies put follistatin inside the hormone system that runs fertility, but none of them tested what an added dose does to it. In female rats, removing the ovaries raised follistatin gene activity in the pituitary, and estrogen replacement fully blocked that rise.10 In pituitary cells grown for the same 2023 study, follistatin on its own did not change the genes for two fertility hormones, LH and FSH.10 When the cells were pushed to make more, it slightly held that rise back.10

In a 2025 mouse study, females missing a gene called Obox1 had smaller litters, and their eggs and ovaries showed more follistatin gene activity.11 A 2022 review lists follistatin among the growth signals made by endometriosis cells, the tissue that grows outside the womb in that disease.12 None of those studies gave follistatin to a living animal and then measured fertility, so they cannot be read as evidence of harm. They are reason enough to leave the question open rather than settled either way.

Does follistatin interact with cancer or cancer treatment?

Mouse studies point in opposite directions here, and the disagreement is itself the useful fact. A 2021 series of mouse ovarian cancer models identified follistatin as a driver of resistance to checkpoint inhibitors, a class of immune-based cancer drugs.13 In that model, follistatin was part of how the tumour escaped treatment.

A 2024 head and neck cancer study in mice used follistatin mRNA to lower activin A, and its treated animals showed minimal metastatic occurrence compared to controls.3 A 2026 lung cancer study in mice reported roughly 2.5-fold greater reduction in tumour burden when its particles delivered the follistatin message to the tumour instead of the liver.4 Which way follistatin pushes depends on the cancer, and these sources offer no way to predict it for a person. The one interaction with a named class of medicine anywhere among them is the checkpoint-resistance finding, and it was observed in mice rather than in patients.

Does follistatin affect the heart or blood vessels?

Only at the level of cells and diseased tissue, and the findings point more than one way. A 1998 review of artery disease reports that in cell experiments follistatin, presumably by blocking activin A, raised a scavenger receptor on immune cells and so promoted cholesterol uptake and foam cell formation.14 Foam cells are immune cells swollen with cholesterol, and they are an early ingredient of artery plaque. The same review found activin A and follistatin present in various atherosclerotic lesions, including human coronary arteries.14

In patients with pulmonary arterial hypertension, which is high pressure in the arteries of the lung, a 2023 study found lower staining for follistatin in lung tissue.15 The same study named activin A and follistatin-like 3, not follistatin, as the prognostic markers in that disease.15 Foam cells in a dish and staining in a diseased lung are not a heart effect measured in a living body. What they do show is that follistatin sits inside artery and lung biology, which is exactly where an outside dose would also arrive.

Who has been given follistatin, and who is missing from that record?

The list is short enough to set out in full. Among the research behind this page, it went to 72 rats with cut or sham-cut nerves, to mice with cancer, and to mice and monkeys in a gene test.25 In people, it went to a small group of patients with inclusion body myositis, a disease that wastes muscle.1 Missing from that list are healthy adults, women and men reported apart, older people with no muscle disease, anyone pregnant, and anyone on other medicines.

The bigger human trials that name follistatin put people on exercise plans instead, and one split 22 women over 65 with age-related muscle loss between weight training and no exercise.16 A finished trial in men with heart disease looked at omega-3 and blood follistatin, and it too measured the protein rather than giving it.17 So every observation about follistatin in a person comes from someone who either had a rare muscle disease or was not given follistatin at all. That describes the record; it is not a reassurance about anyone outside it.

What happens when follistatin delivery stops?

No study among the research behind this page followed an animal or a person after follistatin delivery ended, so there is no withdrawal record to report. The forms used in the research also change what stopping would even mean. In the 2022 rat work the protein came from an osmotic pump placed under the skin, a small implant that seeps its contents out and then runs dry.2

A gene therapy is different, because it sets the animal's own cells producing the protein, and the 2024 delivery paper reports that its follistatin gene raised circulating follistatin levels.5 Its authors present the particles as suitable for redosable gene therapies, meaning a dose can be given again.5 Being able to dose again says nothing about how fast an effect fades once dosing ends, and the summary gives no figure for that.

The nearest thing to an on-off record is exercise: in 60 overweight and obese men, blood follistatin was sampled at baseline and 48 hours after the final session of a 12-week training programme.18 That is a level the body made, sampled once after training ended, and it is not a measure of what follows when an outside supply is withdrawn.

Is follistatin approved, or restricted in sport?

It is not an approved medicine in any record among these sources, and we could find no regulatory action under its name. Drug makers chasing the same muscle signals went another way, and a 2024 review names Bimagrumab, Trevogrumab and Garetosmab as pipeline compounds that block activin and myostatin.19 Those are antibodies, made and tested as their own products, and any safety data they produce belongs to them rather than to follistatin.

A 2013 review of frailty is wary of this whole route, and it says that drugs which block the activin receptor or mimic follistatin-like 3 are less certain because of side effects.20 That caution is about relatives of follistatin, so it cannot be read as a finding about follistatin itself. On sport, none of these sources is an anti-doping test for follistatin. Its status under testing rules cannot be settled from them, so a reader who competes would need the current list from the testing body.

Does a vial sold as follistatin carry risks of its own?

The research cannot size those risks, because none of it used material bought from a seller. The protein that reached animals was made under laboratory control and named by isoform, and the 2022 rat study specifies FS-288.2 The single human study named a different isoform, FS344, and delivered it as a gene carried by AAV1.1 A label reading only follistatin names neither, and the two isoforms are not interchangeable in the research.

The 2022 authors explain why a protein had barely been tried, writing that local delivery of recombinant protein would be more clinically translatable but had yet to be investigated after nerve repair.2 Purity, sterility and identity of retail material appear in none of the studies covered here. That is a supply question rather than a biology question, and it sits on top of a protein whose effects in people are already unmeasured.

Why is the follistatin harm record this thin?

Because most human follistatin research handles it as something to measure in blood, not something to give. In 21 older people with sarcopenia, both light training with blood-flow restriction and conventional heavy training raised blood follistatin over 12 weeks, along with several other markers.21 A 2021 trial in older women reached a cooler verdict, finding only some changes in muscle growth factors such as follistatin and calling the effect of training on them limited.16

A 2023 review of the metabolic literature adds that the exact function of follistatin and its relative follistatin-like 3 has still not been established.22 Trials of this kind report on the safety of a training programme. A harm record for follistatin itself would need people who were given it and then watched over time, and among these sources that has happened once, in a small patient group, using a gene rather than a protein.

What we don’t know

The gaps in the evidence matter as much as the findings.
