Follistatin: what the research measured

Follistatin's measured benefits belong almost entirely to animals given a follistatin gene or mRNA, where muscle grew, grip strength rose and cancer wasting slowed. In the one study that gave rats the protein itself the muscle results were mixed, and among the research behind this page no benefit was measured in a person given a vial.
The strongest signal is a 2024 gene-therapy study in which follistatin DNA, carried in engineered particles, raised blood follistatin and increased muscle mass and grip strength in its animal models. Two mouse cancer studies used follistatin mRNA to slow tumour spread and to protect muscle and fat from wasting.
In people, the record holds one small study in inclusion body myositis, where a follistatin gene delivered by virus was combined with exercise and physical capacity improved alongside exercise levels. The larger human trials measured follistatin rising after training or creatine, which makes it a marker of those interventions rather than a benefit of follistatin.
Follistatin is not an approved medicine among these sources. Each benefit below is tied to a specific preparation and a specific animal or disease, and none of them was measured using material sold under the follistatin name.

What are the benefits of follistatin peptides?

The benefits that have been measured belong to three different objects, and only one of them resembles what a reader can buy. The first is a gene, and a 2024 paper reports that follistatin DNA carried by engineered particles raised circulating follistatin and significantly increased muscle mass and grip strength in its animal models.1 The second is messenger RNA, which in a 2024 mouse study reduced tumour burden and protected muscle fibres from cancer wasting.2
The third is the protein, and in the one study that gave it to rats the authors summarised the result as mixed effects on muscle weight and force.3 So the honest list of benefits is short, and the best entries on it come from methods that rewrite what cells produce rather than from a peptide in a vial. Whether fat loss is among the benefits is taken up on the Follistatin overview, where the answer is that it has not been measured in a person.
What has follistatin been measured to do, and in what model?

Laid out by outcome, the record looks like this. Muscle mass and grip strength rose in mice and non-human primates after systemic follistatin gene therapy, and the same particles kept working when the dose was repeated.1 Satellite cell counts, a measure of the muscle's repair capacity, increased in rats given follistatin protein after nerve injury.3
Tumour spread fell in mice with head and neck cancer given follistatin mRNA, and treated mice showed minimal metastatic occurrence against controls.2 Food intake and body weight were maintained in mice with lung tumours given follistatin mRNA, alongside preserved muscle and fat.4 Physical capacity improved in patients with inclusion body myositis who received a follistatin gene with exercise, in step with how much they exercised.5 Five outcomes, four species and three delivery methods. No two of them were measured the same way, and that is why they cannot be stacked into one claim about what follistatin does.
What does follistatin do for muscle repair?

This is where the protein itself has its one positive result, and it concerns repair cells rather than size. The 2022 rat study opens by noting that follistatin given as recombinant DNA via viral vectors had already been shown to enhance muscle size and satellite cell counts after reinnervation.3 When the team switched to protein, satellite cells still responded, and the authors call the positive effect on the satellite cell population intriguing and worth further work.3
A 2022 mouse study shows the body using follistatin for the same job: after injury, activated support cells in muscle secrete follistatin, which the authors call a promyogenic factor, meaning it favours new muscle.6 In that work, immune cells called M2-like macrophages held back the support cells' follistatin through TGF-beta signalling.6 More repair cells and bigger or stronger muscle are different things, and in the protein study they parted company. The repair signal is real in rats and mice; it is not evidence of faster recovery from training in a person.
Can follistatin protect muscle during cancer wasting?

In mice, yes, and this is the area where the research is moving fastest. The 2024 head and neck cancer study set out to tackle two problems at once, describing its approach as integrating metastasis-suppressing and cachexia-alleviating mechanisms.2 Cachexia is the severe loss of muscle and fat that advanced cancer causes, and it is a different problem from wanting extra muscle.
The 2026 lung tumour study went further on delivery, building particles that gave selective mRNA translation in healthy lungs while favouring protein production in tumours.4 In those mice, the follistatin made in the tumour led to subsequent inhibition of activin A, the signal the authors link to both cancer growth and wasting.4 These are results in sick animals measured against sick animals. Preventing loss in a mouse with a tumour does not tell a healthy reader whether follistatin would add anything to a body that is not wasting.
What has follistatin been studied for in people?

As something given, only inclusion body myositis, and even there the study was built around exercise. The 2018 review that reports the gene study sets it among exercise trials. In the same disease, it notes, weight training with a cuff to limit blood flow kept strength steady, while a group that did not exercise lost a mean of 9.2% of its thigh strength in three months.5 That matters, because exercise alone moved strength in these patients, and the follistatin gene was given with exercise rather than instead of it.
The review's own summary is that exercise could now be considered as medicine in myositis, which is a verdict on training rather than on follistatin.5 Beyond that, follistatin shows up in human work as a blood test, and a 2023 study of 80 patients with high blood pressure in the lungs measured it among a set of related proteins.7 Measuring a protein in patients is not the same as testing it as a therapy, and that gap covers nearly all of the human record.
What has only been shown in animals or cells?

Almost every positive result on this page, which is worth stating plainly before the details. The muscle and grip gains came from mouse and non-human primate models, and the authors describe their particles as having promising potential for redosable gene therapies and genetic medicines.1 The cancer results came from mice, including a murine model of head and neck squamous cell carcinoma, a cancer of the lining of the mouth and throat.2
A weight-loss result that is often attached to the follistatin name belongs to a relative: in obese mice, intranasal delivery of follistatin-like 1 promoted weight loss and improved insulin sensitivity.8 Follistatin-like 1 is described by its own researchers as a signaling molecule that modulates energy metabolism, a separate protein with its own gene.8 A result for a relative is not a result for follistatin, and a result in a mouse has not been shown to carry into a person. Both gaps sit under most of the claims a reader will meet elsewhere.
What are the benefits of follistatin for men?

The evidence specific to men describes what raises a man's own follistatin, not what follistatin does when added. A 2026 review of testosterone explains that the androgen receptor, once bound by testosterone, moves into the nucleus and upregulates follistatin, which blocks signals that restrain muscle growth.9 In that account follistatin is one link in testosterone's chain of effects on muscle, not a replacement for it.
In a 2025 trial of 60 overweight and obese men, every training group gained skeletal muscle and strength while follistatin rose and myostatin fell.10 In a 2024 creatine trial of young adults, both forms of creatine added to training raised the follistatin-to-myostatin ratio along with strength and muscle size.11 In each case the men were given training, creatine or testosterone, and follistatin moved as a result. That tells a reader what lifts the body's supply, which is not the same as showing that a bought supply would add the same muscle.
What are the benefits of follistatin for women?

The research on women is mostly about ageing muscle and the hormones of fertility, and it measures follistatin rather than giving it. In a trial of 22 women over 65 with age-related muscle loss, 16 weeks of weight training improved their fitness, grip, walking speed and strength.12 Yet follistatin and the other growth markers barely shifted, and the main gain the authors stress is that training stopped fat building up inside the thigh muscle.12
A 2024 review of athletes who train hard without eating enough names the hormone axis that includes follistatin as a possible target, while saying its roles are not yet fully worked out.13 That review links the condition to problems such as menstrual dysfunction and bone stress injuries.13 A review naming follistatin as a possible target does not establish a benefit for any woman taking it.
Does follistatin do anything for bone?

The links are indirect, and the most specific one belongs to a related protein. The 1998 review reports that mice lacking follistatin die soon after birth with defects that include skeletal abnormalities as well as poor muscle development.14 That shows follistatin is needed for a skeleton to form normally. It does not show that extra follistatin strengthens an adult skeleton.
A 2024 review of weight-loss drugs describes the myostatin-activin-follistatin-inhibin system as crucial for both muscle and bone maintenance while weight is falling.15 In mice, follistatin-like 1 suppressed the self-renewal of skeletal stem cells, and removing it from nerve cells enhanced bone formation.16 That last result runs against bone, but it concerns follistatin-like 1, so it cannot be read as a finding about follistatin either way.
How long does it take for follistatin to work?

No study among the research behind this page reports a time course for a benefit in a person given follistatin. The rat protein study reported its outcomes after three months of denervation, and the effect it did find, on satellite cells, was reported after the nerve had regrown.3 The gene and mRNA papers report results at their chosen endpoints and give no figure for when a change first appeared.
The human time frames that exist belong to exercise: in 27 older people with sarcopenia, 12 weeks of vibration or resistance training raised blood follistatin.17 Twelve weeks of training is how long the body took to raise its own level in those people. It does not tell anyone how long an outside dose would take, or whether it would act at all.
Does a benefit in one condition carry to another?

Not automatically, and follistatin is a clear example of why. The rats in the protein study had injured nerves, and the authors frame the work around muscle recovery after peripheral nerve repair.3 The mRNA mice had cancer, where activin A was the target because it drives tumour spread and wasting together.2
The 2023 metabolic review warns that even in human disease the outcomes are contradictory and do not allow exact conclusions to be drawn.18 A denervated rat muscle, a mouse tumour and a healthy adult have different things wrong with them, or nothing wrong at all. A benefit measured in one is not evidence for another, and each would need its own test.
Which hoped-for benefits are missing from the record?

Several of the reasons people search for follistatin have no measurement behind them among these sources. Added muscle in a healthy adult, faster recovery from training, athletic performance and fat loss in a person were not measured after follistatin was given in any study covered here. The nearest human finding on fat runs the other way, since a 2021 study reports that elevated circulating follistatin associates with an increased risk of type 2 diabetes.19
The 2022 rat study is the only test of the protein for muscle, and in some of its groups follistatin treatment resulted in smaller muscles.3 That a benefit has not been measured does not mean it has been ruled out. It is a claim with nothing under it yet, and it should be read with exactly that weight.
What would it take to show a benefit in a healthy adult?

A trial of a kind that does not appear among the research behind this page. The 2022 rat authors point the way, arguing that local delivery of recombinant protein would be more clinically translatable than gene delivery if it worked.3 A useful study would give a defined isoform of the protein, at stated amounts, to healthy adults, against a placebo group doing the same training, and measure muscle, strength and blood sugar.
The 2023 metabolic review asks for something similar, stressing the strong need of performing further research to explain follistatin's exact role.18 Until a study like that exists, the gene-therapy animals are the best evidence of what follistatin can do, and they describe a laboratory method rather than a product.
What we don’t know
The gaps in the evidence matter as much as the findings.
- 01Whether follistatin adds muscle to a healthy adult. The muscle gains among these sources came from gene therapy in mice and non-human primates, and the one protein study in rats gave mixed results.
- 02Whether any benefit follows from protein injected under the skin. Every clearly positive result behind this page used a gene or an mRNA message to make the animal's own cells produce follistatin.
- 03How long a benefit would take to appear in a person. The animal studies report outcomes at fixed endpoints, and no human time course exists among these sources.
- 04Whether the cancer-wasting results apply outside cancer. Preserving muscle in a mouse with a tumour and adding muscle to a healthy body are separate questions.
- 05Whether follistatin helps bone directly. The bone links among these sources come from knockout mice, a weight-loss review and a related protein, follistatin-like 1.
