Thymosin Alpha-1

Thymosin alpha-1 is an immune-modulating peptide that, unlike almost everything else on this site, carries a regulatory record: its synthetic form, thymalfasin, has been approved by various agencies for hepatitis B and as a vaccine-response enhancer in immune-compromised people. An approval in one country does not travel to another, so which agencies, and on what terms, decides its worth.

That approval wording comes from a 2023 oncology paper. The strongest human signal sits in chronic hepatitis B. A 2004 review counted seven randomized controlled studies on Talpha1 monotherapy in patients with chronic hepatitis B. Six months of it resulted in a significantly higher sustained response rate than untreated controls. An earlier review puts figures on the same question: the virus cleared in 40.6% of patients given it for six months, against 9.4% of untreated controls.

The clinical record is unusually large for a compound in this category. A 2024 systematic review examined clinical studies involving over 11 000 human subjects in more than 30 trials. Size is not the same as settled. A double-blind trial in 508 patients with severe pancreatitis found that it did not reduce the complication the trial was built to prevent, and a European phase 3 study in hepatitis C missed its main endpoint as well.

Material sold as research-grade thymosin alpha-1 is a different object from a labelled vial of thymalfasin, and the United States position is contested in the literature itself. A 2023 patent survey calls thymalfasin the only FDA-approved thymosine-based drug, while a 2024 review refers instead to the FDA's restriction on Tα1 and 21 additional peptides in 2023. One last thing to keep straight: this is not thymosin beta-4, the molecule sold as TB-500, and the two have separate records.

What is thymosin alpha-1?

Thymosin alpha-1 is a copy of something the body already makes, built in a laboratory and given by injection.

A 2023 paper calls it a highly conserved 28 amino-acid peptide naturally occurring in the thymus1. That is the small gland behind the breastbone where T cells are trained. The same paper says the peptide plays critical roles in T cell maturity and differentiation1.

A 2004 review calls it a 28-amino acid polypeptide produced synthetically but originally isolated from thymosin fraction 52. That fraction is a bovine thymus extract containing a number of immunologically active peptides2.

So thymalfasin, Tα1 and thymosin alpha-1 all name one molecule. A search using only one of them will miss most of the record.

Is thymosin alpha-1 the same as TB-500?

They are not the same molecule. Thymosin alpha-1 and thymosin beta-4 are different peptides that happen to share a surname, and TB-500 is sold as a fragment of the second one.

The names look related because both came out of one starting material. A 2007 review explains that starting with fraction 5, several main peptides (thymosin alpha(1), polypeptide beta(1), and thymosin beta(4)) were isolated and tested for biological activity8.

The functions are not close. The same review calls the beta thymosins the main intracellular G-actin-sequestering peptides in most vertebrate cells8, which means they bind the protein that builds the inner scaffolding of a cell. Thymosin alpha-1 acts on immune cells instead.

A 2026 thyroid review lists thymosin alpha-1, thymosin beta-4, BPC-157, and growth hormone secretagogues9 as four separate entries. The beta-4 record sits on its own page at the TB-500 overview.

Is thymosin alpha-1 approved anywhere?

In a number of countries, yes, and the approval is narrower than the marketing around it suggests.

A 2023 paper records that thymalfasin has been approved by various regulatory agencies for hepatitis B viral infection1. The same line adds a second use, as an enhancer of vaccine response in immune-compromised populations1.

The United States record is messier, and two reviews behind this page disagree flatly. A 2023 patent survey says that Thymalfasin (Zadaxin) is the only FDA-approved thymosine-based drug7. A 2024 review refers instead to the FDA's restriction on Tα1 and 21 additional peptides in 20234.

Both cannot be a full account of one regulator, and that they disagree is itself the useful fact. An approval granted in one country is real but not transferable. It says nothing about how another regulator would read the same evidence.

How far into people has thymosin alpha-1 been studied?

Further than almost anything else on this site, and the size of that record rewards careful reading.

A 2024 systematic review examined clinical studies involving over 11 000 human subjects in more than 30 trials4. The single trials behind it are real. A 2022 pancreatitis trial was multicentre, double-blind and placebo-controlled, and a total of 508 patients were randomised, of whom 254 were assigned to receive Tα1 and 254 placebo5. A 2024 bowel cancer study enrolled 400 patients, allocated by a random number table.12 A 2004 review counted seven randomized controlled studies on Talpha1 monotherapy in patients with chronic hepatitis B2.

What those trials measured is virus counts, immune markers and infection rates in people who were already seriously ill, which is a long way from the reason most readers open a page like this.

What did the hepatitis trials actually find?

Mixed results, with a pattern that favours hepatitis B over hepatitis C.

A 2001 review reports that TA1's efficacy in hepatitis B has been evaluated in 195 patients in four clinical trials3. In the controlled study among those four, the virus cleared in 40.6% and 25.6% of patients treated with TA1 for 6 and 12 months3, against 9.4% of untreated controls. A 2004 review sums up the single-agent work: six months of it resulted in a significantly higher sustained response rate than untreated controls in patients with chronic hepatitis B2.

Hepatitis C is where it mostly did not hold. A 2010 review records the largest miss, from a large phase-III randomized study conducted in Europe6, where thymalfasin did not improve the rate of sustained virologic responses6. It did cut the relapse rate among the patients who finished the course.6 A missed main endpoint with a good secondary finding is weaker than it reads as.

What does the cancer research show?

It is given alongside cancer therapy rather than against a tumour on its own, and the evidence is uneven.

A 2026 review covers trials in lung, liver and skin cancers. It reports potential therapeutic benefits10. It then adds that the available evidence remains limited and heterogeneous10.

The positive signals cluster in the adjuvant setting, meaning the compound is given around surgery or chemotherapy rather than instead of it. A 2023 paper reports that Tα1 could significantly improve overall survival in patients with surgically resectable lung cancer and liver cancers in the adjuvant setting1. In the 2024 bowel cancer study, patients given it had notably lower incidences of perioperative infection than the control group12.

The counterweight is a 2026 hospital review of 120 patients with advanced lung cancer11. There the two standard measures of tumour shrinkage did not differ significantly among groups11. Fewer infections after an operation and a smaller tumour are different things.

Where has thymosin alpha-1 failed?

In at least two places among the research behind this page, and the failures are as useful as the positive results.

The clearest sits in the pancreatitis trial. ANP is severe pancreatitis in which part of the organ dies, and IPN is that dead tissue becoming infected. The trial concluded that the immune-enhancing Tα1 treatment of patients with predicted severe ANP did not reduce the incidence of IPN during the index admission5. It also found no difference in other major complications, including new-onset organ failure, bleeding, and gastrointestinal fistula5.

The second failure was in mice. A 2015 study ran two immunocompetent murine MM models14, meaning mice with working immune systems and implanted myeloma. Its authors write that we did not observe any impact of Tα1 administration on MM development in vivo14. In the same work Tα1 failed to improve immune recovery in a xenogeneic HSCT model14, which used human blood stem cells in mice.

What is thymosin alpha-1 used for?

Approved use and reported use are two different lists, and the second is much longer. The approved list is hepatitis B viral infection, plus use as an enhancer of vaccine response in immune-compromised populations1.

The reported list comes from the same 2023 paper, which records that in China, it has also widely utilized in patients with cancer and severe infections1. It names the emergency use during SARS and the COVID-19 pandemic as an immune-regulator1.

One further line in that 2023 paper earns scepticism rather than trust. It says Tα1 has been shown to restore immune functions and help to reduce mortality in patients with severe sepsis1, without pointing to a trial of its own. A line in the opening of a review and the work it summarises are different things.

How was thymosin alpha-1 given in the studies?

Under the skin, twice a week in the hepatitis work and far more often in acute illness. These are study parameters reported as published, and they are not guidance for anyone.

A 2004 review gives the hepatitis schedule as Talpha1 (1.6 mg twice-weekly)2 across six months, and the 2024 bowel cancer study ran the same amount on two frequencies, twice a week in one arm and thrice a week in the other12. Each arm held 100 patients.12 In the 2022 pancreatitis trial, patients were assigned to receive a subcutaneous injection of Tα1 1.6 mg every 12 h for the first 7 days and 1.6 mg once a day for the subsequent 7 days5.

It does not hang around in the blood, and a 2001 review reports that TA1 is rapidly absorbed, achieving peak serum concentrations within two hours3. Those figures belong to a labelled injection given in hospital, and none of it carries over to an unlabelled vial.

How long does thymosin alpha-1 take to work?

The hepatitis literature answers this directly, and the answer is slower than most readers expect. A 2004 review states that the benefits of Talpha1 therapy is usually not immediately apparent during therapy2, keeping the original author's grammar.

It then describes something genuinely odd. There is a trend for complete virological response to increase or accumulate gradually after the end of thymosin therapy2. In plain terms, the measured effect kept building after the injections had stopped.

Read what that applies to. It is a virus count in people with chronic hepatitis B, tracked across months by a blood test. No study among the research behind this page measured onset of anything in a healthy adult, so the question most readers are asking has not been asked of this compound.

Who should not take thymosin alpha-1?

The honest answer is that the trials record who they enrolled, not who they would keep away. That is a smaller answer than the question wants, and it is the one the published work supports.

Look first at who was studied. The 2022 pancreatitis trial took in patients with an Acute Physiology and Chronic Health Evaluation II (APACHE II) score of 8 or more5. The vast majority of the participants required admission to the intensive care unit (ICU)5. Nobody on that list resembles a healthy adult who has bought a vial online.

The closest thing to a caution among these sources is a 2026 thyroid review, which notes that safety profiles vary across peptide classes, and clinical use remains largely investigational9. An immune modulator given to somebody with an autoimmune condition is the obvious open question, and it is open because it has not been asked.

Can a doctor prescribe thymosin alpha-1?

That depends entirely on where the doctor is, which is what an approval that does not travel means in practice. Where thymalfasin holds a marketing authorisation it is a prescription medicine like any other, and where it holds none, it is not.

The United States position is argued rather than settled. A 2024 systematic review refers to the FDA's restriction on Tα1 and 21 additional peptides in 20234. Its two authors disagree, writing that the restriction appears unfounded4. Their paper then recommends that the FDA permits 503A compounding pharmacies to compound Tα14, which are the pharmacies allowed to mix a preparation for one named patient.

A request printed in a journal is a request and not a rule. That two clinicians had to publish it is itself the useful fact, because it tells a reader the restriction was still standing when they wrote.

What is documented about side effects?

Less than the size of the clinical record might suggest, and most of what exists is local. A 2001 review summarising the hepatitis work records that most studies observed only local irritation at the injection site3.

Cancer studies report harm by comparison instead, which answers a different question. Myelosuppression, the first term in the next sentence, means the marrow making fewer blood cells than it should. In the 2026 lung cancer review, the incidence of AEs, including myelosuppression and gastrointestinal reactions, was comparable across groups11. That does not isolate what the injection does on its own.

A 2020 leukaemia study reports that the most common adverse event was reversible CTCAE grade 2 thrombocytopenia13, a fall in platelets. That study gave chidamide and decitabine plus thymalfasin simultaneously13, so the count belongs to a three-way combination.

What else is in a vial of thymosin alpha-1?

More than one substance, even in material made to a national pharmacy standard.

A 2022 analysis looked at a Chinese Pharmacopoeia reference standard and three commercial materials. It counted twenty-three thymalfasin-related impurities above 0.1 mg/g in the standard alone.15

Those impurities are not dirt off a factory floor. The paper lists them as deamination, amination, succinimide, amino acid insertion/deletion, dimers, and isomers15. More than half of them trace back to a single weak spot. The authors found over half of the thymalfasin-related impurities were found directly or indirectly arising from the labile C-terminal asparagine (Asn) residue15. In plain terms, one end of the chain breaks down on its own.

That study examined the most tightly controlled version of this molecule anyone makes. What sits in an unlabelled research vial was not measured by it.

What is still being tested?

Four registered studies sit among the sources behind this page.

Two are in cancer surgery. One is registered as Thymosin-alpha 1 for Adjuvant Treatment After Radical Resection of High-risk Stage II and III Colorectal Cancer17, and is recorded as recruiting17. The other pairs chemoradiotherapy with a PD-1 inhibitor and thymalfasin in rectal cancer18.

A third is registered as an enhancer of vaccine response among older adults receiving booster doses of COVID-19 vaccine19, and it is a phase 1 study19. The fourth is an older hepatitis C combination study in people who had already failed standard therapy20.

Completed and published are different things. A registry entry marked completed says nothing about what the study found.

How is thymosin alpha-1 thought to work?

By nudging the immune system rather than by attacking anything directly. Natural killer cells are the immune cells that destroy infected or odd-looking cells without being trained on them first.

A 2026 review says Tα1 regulates both adaptive and innate immune responses10. It does so by promoting T-cell maturation and by enhancing natural killer cell activity10, among other routes.

Most of that groundwork was done in the dish. A 2004 review notes that in vitro studies have shown that Talpha1 can influence T-cell production and maturation2.

A 2024 tumour study explains the cancer interest. Tumour-associated macrophages, shortened to TAMs, are the large scavenging immune cells inside a tumour. That study reports that thymosin alpha 1 (Tα1) reprograms “M2-like” TAMs toward an antitumoral phenotype16, meaning it flips them from helping the tumour to attacking it. The work was done in mice and in cultured cells.

What we don’t know

The gaps in the evidence matter as much as the findings.

  1. 01What thymosin alpha-1 does in a healthy adult. Every participant in the trials behind this page had a long-running viral infection, a cancer, or an illness severe enough for intensive care.
  2. 02Whether it does anything a person would notice. The outcomes among the research behind this page are virus counts, immune cell ratios, infection rates and survival, and none of those is a symptom somebody reports.
  3. 03What it does to an autoimmune condition. A 2024 systematic review lists autoimmune disorders among its applications, and the sources behind this page carry no result in that setting for a reader to weigh.
  4. 04Whether the hepatitis B benefit holds up against modern antiviral therapy. The controlled comparisons behind this page run against untreated controls or older interferon regimens.
  5. 05What long-term use does. The longest course among these sources ran twelve months, in people being followed for one named disease.
  6. 06What is in material sold outside the pharmaceutical supply chain. The one purity analysis behind this page examined a pharmacopoeia standard and three commercial materials, and counted twenty-three related impurities in the standard itself.
  7. 07How it interacts with anything else somebody might be taking. The combination studies behind this page pair it with chemotherapy, interferon or antiviral medicines, and no other pairing was tested among these sources.
  8. 08Whether any of the thymosin alpha-1 record transfers to thymosin beta-4 or to TB-500. They are different molecules with separate literatures, and nothing among the sources behind this page reads a result for one as evidence for the other.
