Semax is a research compound whose harm record is mostly silence, because the human studies behind it measured stroke recovery, brain scans and eyesight rather than side effects. Among the research behind this page, no paper reports an adverse event, and that reflects what was collected rather than a count of zero.

The clearest safety-shaped signal is in rats. Given into the nose at 50 and 500 micrograms per kilogram, Semax left anxiety and low-mood measures unchanged in animals that had not been stressed. In dishes of rat brain cells, it did not make glial cells, the support cells of the brain, multiply across a thousandfold range of concentrations. Those are reassurances about rats and dishes, not about people.

The human record is short in time as well as size. The longest exposure on record is a 2018 Russian stroke study of 110 patients given two ten-day courses with a twenty-day gap. Two placebo-controlled studies in healthy volunteers, 24 and 52 people, scanned brains for twenty minutes after a single dose. None of those designs could pick up a rare harm or a slow one.

Russian clinical papers describe Semax as a medicine in use there. A 2026 review that searched FDA and WADA records places it among non-approved peptides, and finds that this group lacks long-term safety data.

How safe is Semax peptide?

Nobody can put a number on it, and the reason is how the research was built rather than what it found. A 2026 review of ageing research searched PubMed, Scopus and the FDA and WADA databases, and its verdict on the unapproved group was that they lack long-term safety data and systematic validation.6 A 2026 orthopaedic review that names semax among the neuroactive peptides puts the whole group in one line: although preclinical studies are promising, there is a current lack of clinical trials.7 Those two lines describe the same gap from two sides. The human work that exists was designed to find benefit in stroke, eye disease and brain imaging, and none of it was set up to count harm. So the honest answer is a description of the evidence rather than a verdict on the compound. A record that never looked for side effects does not tell you whether they happen, and it also does not tell you that they do.

What are the documented side effects of Semax?

None are documented in a person among the research behind this page, and the useful material instead sits in animal and cell work that measured things a reader would worry about. In a Russian rat study, Semax was injected intranasally in doses of 50 and 500 microg/kg 15 min before the testing, and in animals that had not been stressed, Semax administration did not influence the emotional state of animals in the normal state.1 A tenfold step in dose with no shift in anxiety or mood scores is worth knowing, though it is a rat on a maze. In dishes of rat brain cells, Semax did not affect proliferation of glial cells across concentrations from 1 nM to 10 microM, glial cells being the support cells around neurons.2 In stressed rats given daily injections, the forced swim test showed no effects of the CUS procedure or peptides on the duration of rat immobility.8 Each of these is an outcome that did not move. That is not the same as a side-effect record, which needs people, time, and someone writing down what went wrong.

Is Semax nasal spray illegal?

The research behind this page does not settle that, because legality is set by each country and none of these papers is a legal document. What the papers do establish is where the compound sits in medicine. A 2000 Russian ophthalmology paper describes it in its title as a new Russian drug semax, given as nasal drops to patients with optic nerve disease.9 The one nasal study with a placebo group gave its volunteers intranasal 1% Semax (14 subjects) or placebo (10 subjects), so the spray form has been used in published research.4 Outside that setting, the 2026 review that searched FDA and WADA records groups Semax with peptides it calls non-approved, and that group lacks long-term safety data.6 An unapproved compound is not automatically an illegal one, and an approved medicine in one country is not automatically lawful to hold in another. Those are different questions, and a reader needs the law of their own country to answer the second.

Is Semax FDA approved?

No, and the useful part is what an approval would have added. The 2026 ageing review drew the contrast plainly: FDA-approved agents demonstrated robust safety profiles from large-scale trials.6 That sentence describes the evidence an approval rests on, meaning big trials with harm recorded as a primary concern. Semax has nothing of that size. Its largest human study examined one hundred and ten patients after ischemic stroke, split into groups by the timing of rehabilitation and then by whether they got the peptide.3 The same review closes by saying that investigational peptides require rigorous validation through well-designed clinical trials to establish safety and efficacy.6 Russian clinical use and an American approval are different things, and the first does not carry the large safety trials that the second would require.

Is Semax banned in sport?

The papers covered here do not say, and we would rather leave that blank than fill it from memory. The nearest any of them comes is the 2026 ageing review, which ran systematic searches of PubMed, Scopus, and regulatory databases (FDA, WADA) from inception through January 2026.6 Its published summary names Semax among the nine peptides it examined but does not report a prohibited-list status for any of them. So a review that looked at the anti-doping records is not the same as a statement of what those records say. An athlete subject to testing needs the current prohibited list itself, which changes from year to year, rather than an inference drawn from a summary that never stated it. What the research does establish is narrower. The 2026 orthopaedic review files semax with selank and dihexa as neuroactive peptides rather than as recovery or growth compounds.7

What does Semax interact with?

No study among the research behind this page gave Semax alongside another medicine and measured what happened, so there is no interaction list to report. What the receptor work shows is why the question is reasonable. A 2019 rat study found that Semax could modulate in a dose-dependent manner the binding of acetylcholine and GABA to some of their receptors in rats with stress-induced memory impairment.10 A 2023 rat study of related peptides found that each one affected GABA binding in its own way, while preliminary peptide administration did not influence the drop in binding that acute stress caused.11 A 2025 mouse study adds that docking revealed the μ-opioid receptor as a Semax target, which is the receptor family that morphine acts on.12 A shared receptor is a reason to ask about combinations, and it is not evidence of a harmful one. Those are different claims, and only the first is on record.

Does Semax affect how other medicines are broken down?

The only material touching this is a computer model, and Semax was not its subject. A 2017 Russian paper set out to compare mexidol with control molecules (choline alfoscerate, piracetam, glycine, semax) using chemoreactome analysis, a method that predicts activity from chemical structure.13 Its conclusion was that, compared with the four control molecules taken together, mexidol showed a lesser degree of interaction with the potassium channels of the heart, MAO and P450 cytochromes.13 P450 cytochromes are the liver enzymes that clear many medicines, which is why the line catches the eye. Read it carefully, though. The four controls were pooled, nothing was given to an animal or a person, and the abstract assigns no figure to Semax on its own. A modelled comparison in which Semax was one of four reference points cannot be read as a finding that it interferes with drug clearance, or that it does not.

What are the side effects of N-Acetyl Semax Amidate?

No study among the research behind this page has given the acetylated form to a person or an animal, so any side-effect list for it is not drawn from published work. The one relevant paper is laboratory chemistry, and its findings point to the capped molecule behaving differently rather than more gently. It reports that the plain form makes a copper complex that is redox-stable and unreactive against ascorbic acid, unlike the acetylated form, meaning the capped version takes part in reactions the original resists.14 The authors also saw zinc collecting in spots inside nerve cells, which they suggest might explain the zinc toxic effect.14 Their suggested future use for the acetylated form is as a tool to produce a dysmetallostasis in tumor cells, a deliberate upset of metal balance.14 A molecule proposed as a way to disturb cells is not one whose safety can be borrowed from the plain peptide.

What happens when someone stops taking Semax?

No study among the research behind this page followed anyone after the compound was withdrawn and recorded how they felt, so there is no published account of a withdrawal pattern in people. The closest thing is a blood marker that outlasted the dosing. In the 2018 stroke study, standard regimen of semax included 2 courses (6000 mcg/day) for 10 days with 20 day interval.3 The authors report that in those patients semax increased BDNF plasma levels which remained high during the whole study period, BDNF being a protein that supports nerve cells.3 So in those patients the measured change did not vanish in the gaps between courses. In rats, the receptor work speaks of direct and delayed effects, and reports that the delayed effect of stress or preadministration of each peptide on GABA binding was different for its two kinds of binding site.11 A marker that stays raised is not evidence of a rebound or of its absence. Whether dependence can develop is taken up on the Semax overview, alongside the opioid-receptor finding behind that question.

Is Semax hard on the liver or kidneys?

Among the research behind this page, nobody measured liver or kidney function in a person or animal given Semax, so that question has no published answer. The organ work that does exist is on the gut, and it runs in the protective direction. In albino rats, Semax given intraperitoneally, meaning injected into the belly, at 50 micrograms per kilogram an hour before ulcer-causing insults protected the stomach lining from damage.15 In rats held under restraint stress, giving the peptide alleviated stress-induced pathomorphological changes in the colon wall, across doses of 5 to 450 micrograms per kilogram.16 Those are findings about a stomach and a colon under deliberate injury. They say nothing about how a liver handles the compound or how kidneys clear it, which are different questions. An organ protected in a rat stress model is not evidence about one that was never examined.

Is it safe to take Semax every day?

No study among the research behind this page has followed daily use in a person beyond ten days at a stretch, so there is no long-run answer. The animal work on repeated dosing shows that the compound does change things in a healthy brain, which is the reason the question deserves care. In rats given intraperitoneal Semax with no injury at all, sequencing found 258 genes changed in the frontal cortex within a day of the first dose.17 Most of those changes ran one way, since the peptides predominantly caused decrease in expression of the genes associated with the immune system.17 In rats after a stroke, a 2014 study found the reverse, with the peptide predominantly enhancing the expression of genes related to the immune system.18 Down in healthy brains and up in injured ones is a meaningful pattern. It does not tell you what daily use for months would do, and nobody covered here measured that.

How long does the human safety record for Semax run?

At its longest, about forty days, and the course was designed to measure recovery rather than harm. The stroke patients received two ten-day courses separated by twenty days, which is the longest documented exposure among the research behind this page. What that study tracked was plasma BDNF levels, motor performance on the British Medical Research Council scale and Barthel index, a score of how well someone manages daily tasks.3 The healthy-volunteer work is far shorter, with scans taken before, after 5 and 20 min of the injection of either Semax, or Selank, or placebo.5 The 2001 glaucoma report describes a complex of neuroprotective therapy, including a new Russian neuropeptide semax, and its summary gives no duration at all.19 Twenty minutes of scanning and forty days of rehabilitation describe how long people were watched for a benefit. They do not describe how long anyone was watched for a harm, and that is the figure missing here.

Who was excluded from the Semax studies?

The published summaries do not list exclusion criteria, so we can only describe who was included, and the gaps are visible from that alone. The placebo-controlled imaging study enrolled healthy volunteers (11 men and 13 women aged 43.9±9.5 years).4 The stroke study enrolled adults with a mean age of 58, and the eye studies enrolled patients already receiving standard care. Children and pregnant women do not appear as described groups in any of them. One 2013 paper is titled Semax corrects brain dysfunction caused by prenatal introduction of valproic acid, and only that title is available among these sources.20 Read that title carefully. The exposure before birth that it names is to valproic acid, a separate medicine, rather than to Semax, so it cannot be read as evidence about using Semax during pregnancy. A population that was never enrolled is a population about which the record says nothing, in either direction.

Does the vial itself carry risk the compound does not?

Yes in principle, and the published work gives one reason to worry less and one reason to worry more. On the first point, a 2005 review argues that the peptide family Semax belongs to is far more stable than people used to think, with stability quite comparable with those of major pharmacological preparations.21 That is about the molecule falling apart over time. It says nothing about whether a given vial holds the molecule its label names. On the second point, the 2016 chemistry paper shows that adding a cap to the front end modulates the chemical and biological properties of parental peptide, so a vial sold under a related name holds a different substance.14 No analysis among the research behind this page has tested what is in, or how clean, a vial sold as Semax might be. A stable molecule and a correctly filled vial are different things, and the published work covers only the first.

Why is the Semax harm record this thin?

Partly because of who did the work, and partly because of what it was for. Most of it comes from Russian groups writing about a compound they already regarded as a medicine, a point taken up in detail on the Semax overview. A 2025 paper from that tradition frames its interest around natural peptide drugs lacking side effects, which treats the absence of harm as a starting premise rather than a result.22 A 2024 rat study from the same field argues the opposite need, saying that when studying these peptides under disease conditions, it is necessary to take into account their actions under normal physiological conditions.17 A 2025 review of neuroprotective peptides, Semax among them, concludes that future clinical validation and advanced strategies are essential before these molecules become effective treatments.23 A literature that set out to show benefit, inside one research tradition, will be thin on harm. That is a statement about what was asked, and it does not establish that harm is absent.

What we don’t know

The gaps in the evidence matter as much as the findings.

  1. 01Whether anyone given Semax had side effects. The human papers among the research behind this page report recovery, imaging and vision outcomes and do not list adverse events.
  2. 02What it does over months. The longest human exposure on record is two ten-day courses after a stroke.
  3. 03What it does alongside other medicines. Rat work shows it changes acetylcholine and GABA binding, and no study among these sources gave it with any other compound and measured the result.
  4. 04Whether it affects drug-clearing liver enzymes. The only material touching this is a computer model in which Semax was one of four pooled reference molecules.
  5. 05What the liver and kidneys make of it. Among the research behind this page, the only organ studies are of stomach and colon in rats under deliberate injury.
  6. 06What happens after stopping. No study among these sources followed a person or animal after the last dose and recorded how they fared.
  7. 07What the acetylated form does in a body. The only work on N-Acetyl Semax is laboratory chemistry, and it found the capped molecule more chemically reactive.
  8. 08Anything about pregnancy, children or older adults. None of these groups appears among the people studied.
  9. 09What is in a vial sold under the name. No analysis among the research behind this page has tested the identity, concentration or sterility of such material.
