The Wolverine stack (BPC-157 + TB-500) has no safety record of its own, because no study among the research behind this page has given the pair to people and counted side effects. What exists is a thin record for each compound alone, plus one rat experiment that paired them and measured healing rather than harm.
For BPC-157 (body protection compound 157), a 2025 systematic review found that preclinical safety studies showed no adverse effects across several organ systems, and in the next line that no clinical safety data were found. TB-500 (a synthetic fragment of thymosin beta 4) leans on its parent protein, which reached a 96-patient heart attack trial against placebo.
In people, the record for the pair ends at four patients in a knee chart review who were questioned about pain, never about harm. The largest BPC-157 pilot gave twelve women one bladder procedure and documented no adverse events, without a comparison group. Short observation windows in small groups do not constitute a safety profile, and two separate records do not combine into one for a mixture.
Neither compound is approved by the FDA, and a 2026 primer records both TB-4 and TB-500 as banned in sport. The same 2025 review warns that harm may also come from unregulated manufacturing or contamination, which is a fact about the vial rather than the molecules.
Evidence: No study among the research behind this page measured harm from the pair · one rat experiment of the pair, 32 animals, 4 weeks, healing endpoints only · BPC-157 human record is 3 small uncontrolled studies · TB-500 human trials gave the parent protein
What are the side effects of the Wolverine peptide stack?
review
Side effects cannot be listed for the pair, because no study among the research behind this page has administered BPC-157 and TB-500 together to a person and documented what followed. Any list circulating under the Wolverine name is really two separate compound records, combined by the reader rather than by an experiment. For BPC-157, a 2025 narrative review looked across three small human studies and wrote that no adverse effects were reported, but rigorous, large-scale trials are lacking.1 A 2019 review put the animal side in much the same terms, noting that there are few studies reporting any adverse reactions to the administration of BPC 157.2 Read those two statements precisely: a short list of reported harms in small, uncontrolled studies describes what investigators were looking for, which is not the same as a measured absence of harm. The human trials on the TB-500 side administered its parent protein rather than the fragment itself, and that record is examined on the TB-500 safety page.
Is the Wolverine blend safe to take?
human pilot / early trial
There is no published answer for the blend, so the honest reply is a description of what has been measured rather than a yes or a no. A 2025 patent review takes the most generous line on BPC-157, saying it has a desirable safety profile, since only a few side effects have been reported following its administration.3 The grounds the review offers are a count of reports, not a trial designed to detect harm. A 2026 scoping review of six recovery peptides, both halves of this stack among them, warns that these compounds are often perceived as low risk, despite the absence of efficacy or safety data for many emerging peptides.4 The two reviews are not in contradiction: one counts the harms that happened to be reported, the other emphasises how little anybody deliberately measured, and a short list of harms is consistent with either situation. For the pair administered together to people, that measurement has not begun in any work we could find.
Did the one experiment that paired them record any harm?
animal model
Only one study among the research behind this page administered BPC-157 and TB-500 to the same animals, and it was designed to measure tendon repair rather than injury elsewhere in the body. The rats had a surgically divided Achilles tendon repaired, then received one compound, the other, both, or neither for four weeks. The repaired tendons were then stained, and the slides were evaluated using Bonar and Movin scoring systems, two grading scales for how a tendon heals.5 Those scores grade tendon structure; they do not record weight loss, organ damage or mortality, and the published summary reports nothing about any of those outcomes in either direction. The authors ask for more work pending dose-optimization and longer-term studies, which places the harm question in the future rather than in the paper.5 The single paired result therefore describes healing in thirty-two rats, says nothing about what the combination does to other organs, and sits a considerable distance from any person.
How long does the human safety record for the Wolverine stack run?
human pilot / early trial
For the pair, the human record consists of four patients contacted by telephone, months after their injections. In a 2021 chart review at one Florida clinic, patients were asked to rate their pain prior to injection, the length of time the peptides helped ease the pain and the degree to which the injection helped them.6 None of those questions concerned harm, so the four people who received both compounds were never asked about the subject of this page. For BPC-157 alone, a 2024 bladder pilot gave twelve women one injection procedure and reports that no one dropped out of the study, and no adverse events were reported.7 Twelve participants after a single procedure, without a comparison group, represents a short observation window rather than a safety profile. The parent protein of TB-500 went further, through a randomized, placebo-controlled, double-blind trial involving 96 STEMI patients, people being treated for a major heart attack.8 That trial followed participants for approximately three months, the longest controlled human exposure we could find for either compound, but it administered the full protein in hospital, and a fragment self-administered at home is a different question.
What are the long-term side effects of the Wolverine stack?
animal model
Long-term effects of the pair are unknown, and the reason is simple: the longest paired exposure on record lasts four weeks, in rats, after tendon surgery. A 2026 review of peptides for healthy ageing splits the field in two, finding that FDA-approved agents demonstrated robust safety profiles from large-scale trials.9 The other group, which holds both halves of this stack, showed promising preclinical and limited clinical evidence but lack long-term safety data and systematic validation.9 The longest animal follow-up of the parent protein comes from mice, where rhTB4 improved cardiac function and reduced infarct size at 8 weeks post-MI, meaning eight weeks after a heart attack.8 Eight weeks in a mouse with a damaged heart is a measure of repair, and it does not tell anybody what months or years of use would do to a healthy person.
Does the Wolverine stack affect blood sugar or the heart?
human pilot / early trial
Among these sources no study measured blood sugar or heart rhythm in anybody given BPC-157 or TB-500, so this question rests on a warning about a whole class rather than on a finding. The 2026 scoping review that covers both compounds lists cardiovascular complications and metabolic dysfunction such as insulin resistance as risks it documented across the group.4 Read the attribution closely, because the same review pins its clearest heart signal on another compound: some peptides such as MK-677 were associated with significant risks including congestive heart failure.4 MK-677 is not in the Wolverine stack. A risk listed for a group of six compounds is a reason to ask the question of each one, and it is not evidence that either of these two carries it. On the heart, the TB-500 side was studied as a repair agent instead, and in mice a 7-day rhTB4 treatment prevented cardiac dysfunction and fibrosis 28 days post-I/R surgery, where fibrosis means scarring.8 That is a result about repair after an injury, and none of it measured glucose.
Is the Wolverine stack hard on the liver or kidneys?
systematic review
No human study among these sources reports a liver or kidney blood panel for either compound, given alone or as a pair. What is known is the route BPC-157 takes through the body, and a 2025 systematic review records that it is metabolized in the liver, with a half-life of less than 30 minutes, and is cleared by the kidneys.10 Half-life is the time required for the body to eliminate half of a dose; passing through both organs makes them the logical places to investigate, but it does not demonstrate that either is damaged. For the parent protein of TB-500 the kidney work runs the other way, weighing whether it protects the organ across models of acute and chronic kidney injury.11 The same 2026 review proposes conceptual frameworks that consider bidirectional effects on fibrosis, meaning that scarring has been seen to move in both directions depending on the model.11 It also lists completing comprehensive safety assessments among the work still to be done.11 Protecting a kidney in an animal model and leaving a kidney undamaged in a person are different questions, and only the first has been investigated.
What does the Wolverine stack interact with?
review
The first interaction is the pair itself, and no study among the research behind this page measured what BPC-157 and TB-500 do to each other's safety. Combination therapy effects sit on the gap list of a 2026 review of peptides in ageing, right beside dosing and ways of tracking whether anything works.9 With other medicines, one line of rat work deserves a note, because a 2021 review describes BPC-157 counteracting bleeding disorders, produced by amputation, and/or anticoagulants application, anticoagulants being blood thinners.12 The same review says that BPC 157 may prevent and/or attenuate or eliminate, thus, counteract both arterial and venous thrombosis, which means clots in arteries and in veins.12 Those are results in rats. A compound that acts on clotting in rats makes blood thinners an obvious interaction to study in people, and that is a reason to ask the question rather than an answer to it.
Who was excluded from the Wolverine stack studies?
human pilot / early trial
With no human trial of the pair among these sources, there is no exclusion list for it, so the useful answer is who the separate studies took in, and by implication who they left out. The paired rat experiment used thirty-two male Sprague-Dawley rats, each aged 12 weeks, so female and older animals were never part of it.5 The BPC-157 bladder pilot took in twelve women between the ages of 39 and 76 years, all with a single bladder condition and no men at all.7 Pregnancy appears once, in mice, where the parent protein of TB-500 was given on two days of gestation and the authors write that it may act as a powerful fetal growth promoter.13 A compound that accelerates organ development in an unborn mouse is acting on fetal development, which makes pregnancy a genuine question rather than a settled one, and no pregnant participant appears in any study covered here.
What did the thymosin beta 4 safety trials find?
registered trial
Several registered trials of the parent protein carry the word safety in their titles, and their status says more than their names do. One is a Safety and Efficacy Study of Thymosin Beta 4 in Patients With Acute Myocardial Infarction, and its listed status is completed.14 A Phase 1 Safety Study of the Intravenous Administration of Thymosin Beta in Healthy Volunteers, a drip given to healthy people, was withdrawn before it ran.15 A Phase 2 Study on Effect of Thymosin Beta 4 on Wound Healing in Patients With Epidermolysis Bullosa, a fragile-skin disorder, was terminated early.16 An eye-drop study, Safety and Efficacy of Thymosin Beta 4 Ophthalmic Solution in Patients With Dry Eye, is listed as completed.17 A registry entry records that a trial ran rather than what it found, and none of these entries carries a published result among the sources behind this page. Every one of them also gave the full protein, by drip, on wounds or as eye drops, which is not the fragment sold as TB-500.
Does the vial itself carry risk the compounds do not?
systematic review
A 2025 systematic review of BPC-157 says so directly, concluding that adverse effects are possible due to unregulated manufacturing, contamination, or unknown clinical safety.10 A 2025 narrative review points to the compound's wide availability through non-regulated sources, a phrase that says nothing about what any given vial holds.1 Even the bladder pilot named its supply, testing BPC-157 manufactured by a 503A compounding pharmacy, a pharmacy that makes up preparations to order.7 Whatever that pilot did or did not see belongs to that pharmacy's material, and it does not carry over to powder from somewhere else. A vial holding two compounds doubles the number of things that can be wrong with it, and no analysis among the research behind this page has tested what a vial sold under the Wolverine name contains.
Is the Wolverine stack FDA approved, and what does that cover?
animal model
Neither compound is approved by the FDA for any use, alone or together, and for a reader asking about harm that matters as much as the legal point. A 2025 patent review records that BPC-157 has not been approved for use in standard medicine by the FDA and other global regulatory authorities, and it puts that down to missing clinical studies.3 Approval would have come with a safety file, built from trials large enough to catch uncommon harms. A 2003 rat paper described BPC-157 as currently in clinical trials for inflammatory bowel disease (PLD-116, PL 14736, Pliva), and the results of that programme do not appear among the research behind this page.18 On the sporting side, a 2026 primer for orthopaedic surgeons notes that TB-4 and TB-500 both remain banned substances in sports, and the shifting status of BPC-157 is traced on the Wolverine stack comparison page.19 A ban in sport is a rule about competition rather than a finding about harm, and it should be read that way.
Why is the Wolverine stack harm record this thin?
systematic review
Part of the answer is who did the work, since a 2019 review of BPC-157 observes that only a handful of research groups have performed in-depth studies regarding this peptide.2 The 2025 systematic review describes its own evidence as level IV and level V studies, the two lowest rungs on the usual ladder, well below any controlled trial.10 Even the strongest human result on the TB-500 side ends by saying that further rigorous randomized studies are needed to assess the significance of early rhTB4 use.8 A 2026 primer for orthopaedic surgeons sums up the field by saying that significant research regarding the safety and efficacy of these therapeutic methods is required first.19 A thin record documents questions not yet investigated, which is not the same as a record of harms that were searched for and not found; for a two-compound blend, the first unanswered question still concerns the combination itself.
What we don’t know
The gaps in the evidence matter as much as the findings.
- 01Anything about harm from the pair. No study among the research behind this page gave BPC-157 and TB-500 to a person together and recorded adverse events.
- 02Whether the two change each other's effects. A 2026 review of peptides in ageing lists combination therapy effects among its open knowledge gaps.
- 03What either compound does to the liver or kidneys in a person. No human study among these sources reports an organ blood panel.
- 04Whether BPC-157 interacts with blood thinners in people. The clotting findings in these sources come from rats.
- 05What exposure in pregnancy does. The only pregnancy data in these sources is the parent protein of TB-500 given to pregnant mice, where it acted on fetal growth.
- 06What happens past a few months. The longest controlled human follow-up in these sources ran 90 days, with the full protein, in heart attack patients.
- 07What a vial sold under the Wolverine name contains. No analysis among the research behind this page has tested its identity, strength or sterility.
