Sermorelin (GRF 1-29) is compared with half a dozen other compounds in the places it is sold, and with exactly one in the places it is studied. That one is somatropin, which is manufactured growth hormone, and even that comparison was made across separate studies rather than inside a single trial.

The result sits in the 1999 paediatric review. Height velocity gains in children given subcutaneous sermorelin, as a continuous infusion or in three divided doses, were less than in children given once daily subcutaneous somatropin at the same weight-based amount. The same review states that the effects of the recommended dosage of sermorelin have not been directly compared with those of somatropin, which is the qualification that makes the first sentence readable.

Everything else is a family resemblance rather than a measurement. A 2026 review brackets sermorelin with tesamorelin and both versions of CJC-1295 as growth hormone-releasing hormone analogues, puts ipamorelin and the growth hormone-releasing peptides in a second group, and IGF-1 LR3 in a third. BPC-157 belongs to a different family again, grouped with the wound-healing peptides rather than with the growth hormone axis. No study among the research behind this page gave sermorelin and any of them to the same subjects.

Sermorelin's brand was Geref, and a 2026 review records the absence of regulatory approval for physique- or performance-related indications. Growth hormone-releasing hormone and its synthetic analogs are prohibited by the World Anti-Doping Agency. Ten Class II FDA recalls of compounded sermorelin sit on the enforcement record, and several of those vials also held ipamorelin or a growth hormone-releasing peptide.

Evidence: One indirect comparison, against somatropin, in children with a diagnosed deficiency · no head-to-head trial against HGH, CJC-1295, tesamorelin, MK-677, IGF-1 LR3 or BPC-157 among the research behind this page · family groupings from three 2026 reviews · ten Class II FDA recalls, several of them combination vials

What has sermorelin actually been compared against?

review

Once, and indirectly, against the hormone it asks the body to release. Everything else sermorelin gets lined up against has never been put into the same experiment with it among the research behind this page.

The 1999 paediatric review is the only place a rival compound appears beside it, and the review is careful about what that was. The effects of the recommended dosage of sermorelin have not been directly compared with those of somatropin, which is manufactured growth hormone.1 What the review offered instead was a reading across separate studies, and that is a weaker instrument than a trial with two arms in it.

Every other pairing on this page is a market pairing rather than a measured one. A 2026 review brackets it with growth hormone-releasing hormone (GHRH) analogues (e.g., sermorelin, tesamorelin, CJC-1295 with Drug Affinity Complex [DAC], CJC-1295 without DAC), and appearing on one list together is not the same as being tested against each other.2

Sermorelin vs HGH: where do they differ?

review

One supplies the hormone and the other asks for it, and every practical difference follows from that. Endogenous means made by the body itself.

Sermorelin is the signal rather than the cargo. A 2003 review reports that it maintains bioactivity in vitro and is almost equally effective in eliciting secretion of endogenous growth hormone in vivo, so what reaches the blood is the person's own hormone released on the pituitary's own pattern.3 HGH is the hormone itself, injected whole, and what its approvals cover is set out on the HGH page rather than here.

The second difference explains why anybody built the comparison. The 2017 chart review states its own motive in its first line: realizing the reported misuse of human growth hormone (GH), investigation of a safe alternative mechanism for increasing endogenous GH is needed.4 That is a reason for running a study rather than a result from one, and no study among the research behind this page has since set the two against each other.

What happened when sermorelin was measured beside somatropin?

review

The children on sermorelin grew more slowly than the children on the hormone itself. That is the most load-bearing comparison in the entire record, and it arrives with two qualifications attached.

Height velocity gains in children given subcutaneous sermorelin daily, as a continuous infusion or as three divided doses, were less than those in children receiving once daily subcutaneous somatropin at the same weight-based amount.1 Somatropin is the manufactured form of growth hormone.

Now read the qualifications. The review states that the effects of the recommended dosage of sermorelin have not been directly compared with those of somatropin, so this is a reading across separate studies rather than a single trial with two arms.1 And every child in it had a diagnosed deficiency, which makes the question whether a missing signal can be replaced. An adult with a working pituitary is asking something else entirely.

Sermorelin vs CJC-1295: how close are they?

review

Same family, different staying power, and nothing among the research behind this page sets them against each other. A 2026 review puts them in one bracket, listing sermorelin, tesamorelin, CJC-1295 with Drug Affinity Complex [DAC], CJC-1295 without DAC as versions of the same idea.2

The difference that matters is how long each one lasts. Sermorelin has a short half-life in plasma, about 10-20 min in humans, which means the kidneys clear it and enzymes clip its front end off within the hour.3 The longer-acting members of the family exist because of exactly that, and a 2003 review describes PEGylation as one valid approach to obtain more stable forms of the peptide, with a longer in vivo half-life.3

What none of that settles is whether lasting longer produces a better result in a person. No experiment among the research behind this page measured an outcome for sermorelin and an outcome for CJC-1295 side by side, so the comparison rests on chemistry rather than on anything anybody observed. What the CJC-1295 studies measured is set out on its own pages.

Sermorelin or tesamorelin: what separates their records?

animal model

Neither has been set against the other among the research behind this page, so what separates them is paperwork rather than a result. A 2026 sports medicine review surveys approved and unapproved peptides marketed direct to patients and names tesamorelin by its brand, writing it as tesamorelin (Egrifta), while sermorelin appears in the same list with no brand beside it.5

A doping-control study puts the same pairing in the laboratory rather than the clinic. Its target analytes included Geref (Sermorelin), CJC-1293, CJC-1295, and Egrifta (Tesamorelin), which tells you the two behave alike enough to be hunted with one method.6

Which review counts as approved is not spelled out compound by compound, and the sources behind this page record no current approval for sermorelin in any adult indication. Which of the two does more for a person has not been asked in any experiment among the research behind this page, and a page answering it would be supplying a verdict the studies do not contain.

Sermorelin vs BPC-157: are they the same kind of compound?

review

They are not, and one sentence from a 2026 orthopaedic review is the clearest answer there is. Angiogenesis means growing new blood vessels. Fibroblasts are the cells that lay down scar and connective tissue.

Wound-healing peptides such as BPC-157, TB-500, and GHK-Cu promote angiogenesis, integrin-mediated extracellular matrix remodeling, and fibroblast activation, whereas growth hormone secretagogues like ipamorelin, CJC-1295, tesamorelin, sermorelin, and AOD-9604 activate IGF-1 signaling and satellite cell repair.7 Satellite cells are the muscle's own repair stock.

So the two sit on different sides of one sentence, doing different jobs. A 2026 sports medicine review puts them in one bucket for a different reason. It surveys peptides marketed direct to patients, including AOD-9604 (anti-obesity drug 9604), BPC-157 (body protection compound 157), CJC-1295, with sermorelin further down the list.5 Whether anyone gives the two together is a separate question. The record on that pairing sits on the BPC-157 comparison page.

MK-677 vs sermorelin: what has been published on the pair?

review

MK-677 is written as ibutamoren in the literature, and that is the name to search for. One 2020 review covers both compounds, and no study among the research behind this page has compared them.

The review's own list is the connection. The GHS that will be discussed include sermorelin, growth hormone-releasing peptides (GHRP)-2, GHRP-6, ibutamoren, and ipamorelin.8 GHS stands for growth hormone secretagogues, meaning compounds that prompt the body to release its own growth hormone.

It then writes a single judgement across the whole group, in hypogonadal men. All are potent GH and IGF-1 stimulators that can significantly improve body composition while ameliorating specific hypogonadal symptoms including fat gain and muscular atrophy.8 Read the same review's next sentence before doing anything with that one, because a paucity of data examining the clinical effects of these compounds currently limits our understanding.8 A judgement written across five compounds at once describes a group rather than either of the two compounds anybody came here to tell apart.

IGF-1 LR3 vs sermorelin: do they sit in the same category?

animal model

They do not, and that difference is most of the answer. A 2026 review sorts these compounds into families, and IGF-1 LR3 lands among insulin-like growth factor-1 (IGF-1) analogues (e.g., pegylated mechano growth factor (PEG-MGF), IGF-1 Long R3 (IGF-1 LR3)), two steps along the chain from where sermorelin sits.2

The chain runs in order. Sermorelin asks the pituitary for growth hormone, growth hormone drives the liver to make IGF-1, and insulin-like growth factor 1 serves as a surrogate marker for GH in the studies that measure anything at all.4 An IGF-1 analogue skips both steps and supplies the end product directly.

Doping-control chemistry treats them as separate problems as well, running growth hormone-releasing hormones in one group and insulin-like-growth factors (long-R3 -IGF-I, R3 -IGF-I, des1-3 -IGF-I) in another.9 Nothing among the research behind this page gave one and then the other to the same subjects, so there is no measured comparison to report rather than a close one.

Does sermorelin raise IGF-1 the way its comparisons assume?

primary research

One dataset says yes, and it cannot say it about sermorelin on its own. The 2017 chart review's own conclusion is that GHRP/SERM therapy increases serum IGF-1 levels with strict compliance to thrice-daily dosing in those men, where SERM is sermorelin and the two GHRPs are separate peptides given at the same time.4

That matters for every comparison on this page. A number produced by three compounds together cannot be set beside a number produced by one compound alone, and the other compounds people want ranked against sermorelin were studied one at a time or not at all.

The authors are careful about where the result points in human terms: the results suggest that combination therapy may be beneficial in men with wasting conditions that can improve with increased GH secretion.4 Suggest, combination and wasting conditions are each doing work in that sentence, and none of them describes a healthy adult choosing between two vials.

Which compounds appear beside sermorelin in the recall record?

regulatory action

Growth hormone-releasing peptides, over and over again. The enforcement record holds a Sermorelin Acetate/GHRP (2) 9 mg/3 mg Injectable Kit from one pharmacy, a Sermorelin/GHRP-2 18 mg/6 mg powder and a Sermorelin/GHRP-2 9 mg/9 mg Powder from a second, and a Sermorelin/GHRP-6/GHRP-2 Injection 3MG/3MG/3MG from a third.10111213

Read what that is and is not. A recall notice records what a compounding pharmacy was selling, which is a fact about supply rather than a finding about a combination. Every one of those lots came off the shelf over the sterility of its production rather than over the pairing inside the vial.13

The pattern is still worth having, because the partners sitting beside sermorelin in that record are the same names the 2020 and 2026 reviews group it with. The pharmacies and the literature agree about which family this compound belongs to, even where no experiment among the research behind this page compares its members.

Is sermorelin stacked with ipamorelin?

review

Different brackets, same shelf. A 2026 orthopaedic review groups ipamorelin, CJC-1295, tesamorelin, sermorelin, and AOD-9604 together as growth hormone secretagogues.7 The 2026 review of the same axis splits that bracket in two. It files ipamorelin under growth hormone secretagogues (GHS; e.g., growth hormone-releasing peptide-2 (GHRP-2), growth hormone-releasing peptide-6 (GHRP-6), hexarelin, ipamorelin), and sermorelin elsewhere.2

The practical difference is which door each one knocks on. Sermorelin's own family binds and activates the human GRF receptor, the one the brain's own releasing hormone uses.3 The second review keeps the other group apart from it.

Nothing among the research behind this page gave both to the same subjects. What does exist is a withdrawn vial holding both at once, listed as SERMORELIN ACETATE 6MG/IPAMORELIN 6MG - LYOPHILIZED.14 That documents a pharmacy selling the pairing rather than anybody studying it.

Can a laboratory tell these analogues apart?

animal model

Only just, in one case, and that case shows how close these molecules are to each other. Two molecules can hold exactly the same parts in mirror-image order, the way a left hand mirrors a right, and chemists call that chirality.

A 2023 method paper reports separating, for the first time, enantiopeptides (sermorelin and CJC-1293) that differ by the chirality of only one amino acid.15 Sermorelin and CJC-1293 are that close together.

Everything else is routine by comparison. A 2023 urine method covers tesamorelin, CJC-1295, sermorelin (GRF 1-29), sermorelin (3-29)-NH2, somatorelin in a single run, and a 2026 urine method groups sermorelin/CJC-1293, tesamorelin, and CJC-1295 the same way.1617 Being catchable with one method is a fact about chemistry rather than about what any of these compounds does inside a person.

Where does sermorelin sit on an evidence tier list?

review

Between the two ends of a scale one review draws explicitly. It sorts the compounds it covers by stratifying peptides into evidence tiers from regulatory-grade randomized trial data to a complete absence of human studies, and sermorelin sits at neither end of it.2

What it has is a 1999 review of children with idiopathic growth hormone deficiency, which is genuine clinical data about a population almost nobody searching for it belongs to.1 What it lacks is any adult trial at all. A 2026 orthopaedic review puts the class position in one line, finding that although preclinical studies are promising, there is a current lack of clinical trials.7

Against the other compounds on this page, that leaves sermorelin somewhere odd rather than somewhere strong. It holds real clinical evidence, and none of that evidence is about the people asking. The 2026 axis review draws the same conclusion across the group, highlighting the resulting uncertainty around putative performance and recomposition benefits.2

Why are head-to-head comparisons so rare in this group?

review

Because the studies that exist were never built to answer the question people are asking. Supraphysiological means more of something than a body would ever make on its own.

A 2026 critical review says so directly. Published work, it notes, mostly looks at therapeutic uses under controlled dosing regimens rather than at the supraphysiological or combined protocols common in bodybuilding.18 A trial run to manage a diagnosed condition has no reason to carry a second research compound as a comparison arm. None of these compounds is approved, so nobody is obliged to run one.

The same review adds the plainest line in this whole area: despite their growing popularity, the clinical evidence supporting peptide use in sport is limited.18 So the literature holds single compounds given to patient groups, while the questions people type hold two at a time. Every comparison on this page is a comparison of records rather than of results.

What we don’t know

The gaps in the evidence matter as much as the findings.

  1. 01How sermorelin compares with HGH in an adult. The one comparison among the research behind this page was read across separate paediatric studies rather than run as a trial.
  2. 02Whether a longer-acting analogue does more. No experiment among the research behind this page measured sermorelin and CJC-1295 side by side on any outcome.
  3. 03Which of sermorelin and tesamorelin does more for a person. The two appear in the same lists and in the same laboratory methods, and in no shared experiment among these sources.
  4. 04What MK-677 adds or replaces. Ibutamoren and sermorelin share one 2020 review, which writes a single judgement across five compounds without separating any two.
  5. 05What sermorelin does without GHRP-2 and GHRP-6. The only adult IGF-1 result among these sources came from all three given together.
  6. 06What any combination does. The pairings on record come from recall notices, which document what a pharmacy sold rather than what anybody measured.
  7. 07How any of these compounds compares with doing nothing. No placebo-controlled adult trial of sermorelin appears among the research behind this page.
