hCG is human chorionic gonadotropin, the hormone a placenta makes in pregnancy, and the short version is that it is a prescription medicine with a genuine clinical record that covers almost none of the reasons people search for it.
The deepest evidence sits in fertility clinics. A 2025 review pooled twelve high-integrity RCTs comprising 1,931 women, comparing hCG with other ways of ripening eggs before collection, and the other options did not beat it on pregnancy rates. In men the published work is much smaller and aimed at a different thing. Thirty men with chronic spinal cord injury were given hCG on two consecutive days, at 400, 2000 and 4000 IU, as a test of whether their testes would answer.
Both bodies of work are clinical, both tied to a diagnosis, and both measure something narrow. One counts pregnancies after a single timed injection. The other reads blood testosterone across three mornings. Neither measured body weight, body fat, muscle size or strength, and no study among the research behind this page did either. The weight-loss use shows up in these sources only inside a 2012 toxicology review, which listed human chorionic gonadotropin hormone among dieting agents reviewed for potential toxicity.
The practical reality is a pharmacy medicine with an enforcement history. FDA recall records describe hCG vials as Rx only, and ten recalls of compounded hCG sit in the same file, including one for presence of mold confirmed by outside laboratory at the 14 day culture.

What is hCG?

hCG stands for human chorionic gonadotropin, and the organ that makes it is a placenta. A 2025 review describes that organ as a dynamic endocrine organ that plays a crucial role in fetal development by secreting a diverse array of peptide hormones, with hCG named first among them.1 Production starts before the placenta properly exists. A 2022 review records that classical hCG is the first molecule synthesized by the embryo, built early enough that its RNA is transcribed as early as the eight-cell stage.2 That same review calls it a multi-effect hormone, which is a fair label for something that touches pregnancy, the ovaries, the testes and the thyroid.2 Almost everything medicine knows about hCG was learned in pregnancy, and that shapes every other section on this page.
Is hCG a peptide?

Almost everyone arrives here having typed hcg peptide, and that word is carrying more weight than it can hold. The 2025 placental review does file hCG among peptide hormones, so the search term is loose rather than inaccurate.1 What it conceals is that hCG is not a single short chain of the kind usually marketed under that heading. The 2022 review counts four major isoforms: classical hCG, hyperglycosylated hCG, free beta subunit, and sulphated hCG.2 An isoform is a variant of one hormone with a slightly different construction, and a laboratory has to decide which of them it is measuring. The distinction that matters most is regulatory rather than chemical. hCG has pharmacy versions, a measured record in people and an enforcement file behind it, and that is a different starting point from a research compound that has been through none of them.
Is hCG a prescription medicine?

In the United States it is, and the enforcement file states it in two letters. FDA recall records describe Human Chorionic Gonadotropin 5,000 IU vials, Rx only, and a second entry covers Human Chorionic Gonadotropin 2,500 Units, Vial-Lyophilized, For SC Use, Rx Only.2327 A third covers an oral presentation, listed as HCG (Human Chorionic Gonadotropin) 125 units/ 0.1 mL drop, 5 mL bottle, Rx only.25 Rx only means the vial is dispensed against a prescription written for a diagnosis. Note the limit of what that establishes, because it shapes the rest of this page. Those records are enforcement documents rather than approved labelling, so prescription status is a regulator's own fact here, while the conditions hCG is given for have to be attributed to the clinical literature that describes them.
What is hCG used for in medicine?

Three settings, in the literature behind this page. The first is fertility care. A 2025 analysis compared hCG with three other ways of triggering egg maturation in women having IVF.5 A trigger is the timed injection that ripens eggs before they are collected. The second is undescended testicles in boys. One review states that fixed inguinal, retractile or abdominal testicles may be primarily treated with human chorionic gonadotropin (HCG), and that the success rate after medical treatment averages 50%.8 A second review of the same condition reports that hormone therapy of this kind has been used in the past decades with variable results.9 The third is low testosterone in men, which has a section of its own below. All three sit inside a clinic, attached to a named diagnosis.
What does the strongest human evidence show?

It comes from fertility clinics, and it is substantial. A 2025 review pooled twelve high-integrity RCTs comprising 1,931 women.5 Each woman was randomised to one trigger or another before her eggs were collected. The authors concluded that the other options did not beat hCG on clinical pregnancy rates.5 That is the plainest statement of effect anywhere in these sources. One safety finding ran the other way, with rates of ovarian hyperstimulation significantly lower with the GnRH agonist compared with hCG trigger.5 That syndrome is an over-response of the ovaries, and it is the main documented hazard of this use. Read what the whole body of work is about, though. It counts pregnancies after one timed injection in a watched cycle, and it measures nothing about weight, fat or muscle.
Does hCG raise testosterone in men?

In one published test it did, and the design of that test is the interesting part. Thirty men with chronic spinal cord injury, alongside able-bodied comparison groups, were given hCG at three dose concentrations, 400, 2000 and 4000 IU, by intramuscular injection.4 The hCG was administered on two consecutive days, and blood was collected for serum testosterone in the early morning before each injection, with a final sample on day three.4 The mean serum testosterone concentrations in all groups demonstrated an adequate response.4 So the testes answered the call in every group tested. What the authors were doing, though, was a diagnostic stimulation test, built to find out whether the testes could respond at all. The outcome measured was a hormone reading across three days, and the study ended there.
Is hCG the same as testosterone replacement?

No, and these sources set the two side by side without ever putting them in one experiment. A 2020 review of low testosterone in men says that many therapeutic choices are currently available, depending on the timing of hypogonadism onset and fertility issue.3 Hypogonadism is the clinical word for a body making too little testosterone. The evidence that review gathers covers four options: testosterone itself, hCG, and two kinds of pill that act on oestrogen.3 They are separated there by when the problem started and by whether a man wants children. No study among the research behind this page compared hCG against testosterone replacement in the same group of men. Being listed as options together is not the same as having been measured against each other, and only the first of those has happened here.
Will hCG make a man bigger?

Nothing in this record answers that, and it is worth being exact about why. No study among the research behind this page measured muscle size, strength or body composition in anyone given hCG. What was measured in men is a blood hormone over three days, in thirty men with spinal cord injury and their comparison groups, as a test of whether the testes could respond.4 A hormone reading moving across a weekend and muscle changing across months are different questions, and only the first one has been asked here. One related observation does sit in these sources. A review of breast enlargement in adolescent boys notes that human chorionic gonadotropin and luteinizing hormone receptors on male breast tissue may play a role, and advises taking a history that asks about anabolic-androgenic steroids.11
What about the hCG diet?

This is a large share of what gets searched, and the thinnest part of the record by a distance. hCG appears in a weight-loss setting exactly once in these sources, in a 2012 toxicology review where several different dieting agents were reviewed for potential toxicity.10 The list it belongs to is instructive: syrup of ipecac, cathartics, human chorionic gonadotropin hormone, 2,4 Dinitrophenol, guar gum, phenylpropanolamine and others.10 The framing throughout is harm rather than effect, and the authors write that healthcare providers need to be aware of the potential toxicities of a wide range of weight loss agents.10 No study among the research behind this page measured weight or body fat in anyone given hCG, and no regulatory document among the sources behind this page states a position on the diet. Note exactly what the review does establish. It records that people take hCG for weight loss, which is a fact about behaviour rather than a measurement of effect, and the measurement has not been made here.
How often was hCG given in the published studies?

Twice, in both cases tied to an event rather than to a calendar. In the male stimulation test, hCG was administered on two consecutive days, and the blood samples that mattered were drawn on the mornings around those injections.4 In a 2017 fertility cohort of 74 women, a trigger was used for ovulation with the concomitant use of 1500 IU of urinary human chorionic gonadotropin (hCG) immediately after oocyte retrieval, followed by fresh embryo transfer and standard luteal support.6 Neither of those is a repeating schedule. One is a two-day diagnostic procedure and the other is a single injection timed to an egg collection, and both amounts belong to those designs rather than to a person. What is hCG measured for?

Most of what hCG does in medicine is get measured rather than given. The home pregnancy test is built on it. A 2011 paper describes a strip test for human chorionic gonadotropin, the hormone used to detect pregnancy.12 A 2025 group pointed a phone camera at the same kind of strip. Their video-based method achieved a sensitivity of 92.7%, specificity of 99.1%, and overall accuracy of 98.4%.13 In cancer care the hormone works as a tracking number. A review of placental tumours says the share of patients cured now exceeds 90%, and credits part of that to the ability to monitor therapy effectively with the use of human chorionic gonadotropin as a tumor marker.14 It is watched again after an ectopic pregnancy is managed with medicine, where hCG levels are most predictive of success.15 It also turns up in the marker sets used to flag ovarian masses in young patients.16
What is documented about side effects?

One paragraph, because the detail belongs on the safety page. The documented hazard of the fertility use is ovarian hyperstimulation, the over-response described above. In a 2017 cohort, a low dose of hCG in GnRH agonist trigger cycles caused moderate/severe OHSS in 2.7% of the patients.6 OHSS is the short form of that syndrome. Across the pooled randomised trials, the rate ran lower with the other trigger than with hCG. A 2004 review traces part of the path, noting that several inflammatory markers showed a significant increase after human chorionic gonadotropin (hCG) administration in vivo.7 All of those numbers come from women having their ovaries stimulated. Why does hCG turn up in thyroid medicine?

Because the hormone leans on the thyroid when there is a lot of it about. In early pregnancy, rising hCG can push the thyroid into overproduction, a state clinicians call thyrotoxicosis. A 2013 review reports that this transient gestational thyrotoxicosis is mediated by high circulating concentrations of human chorionic gonadotropin, and that it appears most often in women with severe pregnancy sickness.19 A 2016 review lists hCG among the recognised causes, naming early pregnancy and placental tumours as the settings.20 A 2017 review goes further, saying that high or low hCG levels seem to have a more prominent role in the interpretation of thyroid dysfunction than previously thought.21 Note where all of that was observed. It was observed in pregnancy, where hCG climbs on its own to concentrations that no injection in these sources came close to.
What has gone wrong with compounded hCG?

Ten recall records for hCG injections sit in the FDA enforcement file behind this page, and they are worth reading as a group. One was filed for non-sterility, with presence of mold confirmed by outside laboratory at the 14 day culture, and it carries the agency's most serious recall classification.22 Several more were filed for lack of sterility assurance at compounding pharmacies in Texas and Florida.23 One cites CGMP deviations: lack of potency testing, meaning nobody had confirmed how much hormone was actually in the vial.24 Another describes vials compounded with an incorrect solvent, and two more were pulled over beyond-use dates on the label.2526 Every one of those faults is about how a vial was filled, not about the hormone inside it. That is the line between a pharmacy version and a compounded one, and it is the line this page keeps separate throughout.
Has hCG been studied for anything unexpected?

Yes, and in the opposite direction from everything above. Researchers have spent decades trying to block hCG rather than supply it. A 1994 review states that the antifertility vaccine that will most likely be applied first in family planning programmes is one directed against the pregnancy hormone hCG.17 A 2005 review of the same field records that the hCG vaccine is the first vaccine to undergo phase I and II clinical trials in humans, and that vaccines targeting gamete outcome primarily focus on the hCG molecule.18 That programme is a useful corrective. A hormone central enough to pregnancy that researchers tested blocking it as a contraceptive is not a minor signal in the body.
How does hCG work?

It is a messenger that tells a gonad to get to work, and the detail differs by sex. In men the signal lands on the testes. That is why a stimulation test can quantify testicular responses to the intramuscular administration of hCG and read the answer in blood testosterone.4 In women the target is the ovary. A 2004 review proposes that hCG stimulates the ovaries to produce and secrete a still unknown intermediate factor, which in turn sets off inflammation and may lead to an increase in capillary permeability.7 Capillary permeability is how leaky the smallest blood vessels are, and that second step is how a fertility trigger becomes a documented hazard. Beyond the gonads, a 2022 review lists hCG levels, angiogenetic activity, immunological actions, and effects on miscarriages and thyroid function.2 Angiogenetic means to do with the growth of new blood vessels.
