Ipamorelin protocols in the published literature

No established dose of ipamorelin exists, because no regulator has approved it for anything. The human amounts on record are infusion rates from a 1999 study in healthy men and 0.03 mg/kg twice daily by drip in a 2014 trial after bowel surgery.

The 1999 study gave five rising amounts, from 4.21 to 140.45 nmol/kg, each as a single 15-minute infusion into a vein, to eight men per level. The 2014 trial gave infusions twice a day from the first day after surgery to the seventh, or until discharge, and did not beat placebo on its main outcome.

Read where those figures come from: every human amount was infused into a vein, in a clinic or a hospital, under supervision. No study among the research behind this page gave ipamorelin under the skin, which is how it is usually used, and the animal amounts were set for rats, pigs and fish rather than for people.

What a reader can work out with confidence is arithmetic. The FDA recall record shows vials of 3 mg, 9 mg and 15 mg, and turning a vial and a volume of water into a concentration is a calculation, whereas choosing the amount to draw is the part no study among these sources supplies.

What amounts of ipamorelin did the published human studies use?

Two sets of figures exist, in different units and for different ends. The 1999 study set out to track how the body takes up and clears the molecule. It used 5 different infusion rates (4.21, 14.02, 42.13, 84.27 and 140.45 nmol/kg over 15 minutes)1. The highest amount was more than thirty times the lowest.

The 2014 trial set out to test whether ipamorelin sped up bowel recovery after surgery. Its active arm received intravenous infusions of 0.03-mg/kg ipamorelin vs placebo twice daily2, with the comparison group getting a dummy infusion on the same schedule.

Those are the only human amounts among the research behind this page. One is a ladder of single doses meant to map blood levels. The other is a short course aimed at one problem after surgery. Neither was picked with a healthy adult in mind, and the second did no better than placebo on its main goal, so it is a tested amount rather than a proven one.

The only human amounts of ipamorelin in the studies described above, both given into a vein. Amounts studied, not a dosing guide.

Who received those amounts?

Two narrow populations, neither of them typical of the people who ask this question. The 1999 study enrolled eight healthy male subjects at each dose level1, forty men across the five rates, each receiving a single infusion followed by repeated blood sampling.

The 2014 trial enrolled surgical patients: one hundred seventeen patients were enrolled2, and 114 of them were included in the safety analysis. The setting is stated plainly in the paper: the settings include hospital inpatients.2 Those were people recovering on a ward after part of their bowel had been removed, with nursing staff administering the infusions.

So the human figures come from healthy men undergoing one infusion and from hospital patients in the week after major surgery. An amount selected for either group carries that context with it, and it does not transfer to a person injecting at home, whose physiology, setting and purpose all differ from both.

Is there an ipamorelin peptide dosage per day?

Only one daily schedule has been administered to people, and it consisted of two intravenous infusions a day in hospital. The 2014 protocol ran on postoperative day 1 to 7 or hospital discharge2, so each patient received two infusions a day for a week at most.

That schedule addressed a digestive question, and it is the only per-day pattern in the human record, because the 1999 study gave each man a single infusion and therefore contains no daily figure at all.

The wider literature confirms the gap rather than closing it. A 2026 primer for orthopaedic and sports physicians, which covers ipamorelin, states that information regarding the indications, dosing, frequency, and duration of treatment remains unknown3. Frequency appears in that list, and frequency is exactly the per-day question. A daily amount printed elsewhere did not originate in a study of daily use in healthy people, because no such study appears among these sources; that is different from saying every such figure is wrong, but it means none of them can be verified.

Is there an ipamorelin peptide dosage for women?

No published amount was set for women, and the healthy-volunteer study enrolled none. Its participants were described as healthy male subjects, so every figure for how a healthy body handles ipamorelin comes from men.

The 2014 trial enrolled adults having bowel surgery, and its summary does not break results down by sex. It does report that demographic and disease characteristics were balanced between groups.2 Balanced between the two arms is not the same as analysed separately for women.

The animal work does not help either. The fish study was about the male reproductive system, testing ipamorelin acetate (IPA), a ghrelin agonist, on the reproductive axis of the tilapia Oreochromis mossambicus4 through its effects on the testes. So the question of a dose for women has not been asked, and a figure presented as one would be a guess dressed as a finding rather than a study result.

Do you need to cycle off ipamorelin?

No study among the research behind this page tested a cycle, a break or a taper, so there is no published answer, and the longest human course was a single week in hospital, which is shorter than most cycles people describe.

What the record does show is the shape of a single dose. In the volunteer study, the time course of GH stimulation by ipamorelin showed a single episode of GH release1, a single pulse that subsided. That pattern differs from compounds that keep hormone levels elevated for days, and it is one reason the concept of cycling, borrowed from those compounds, may not correspond to this one.

The genuine unknown is whether the response weakens with repeated use, which is the usual justification for cycling. Answering it would require repeated doses over weeks with blood tests along the way, and no study of that design appears among the research behind this page. The honest position is that cycling has not been investigated, rather than investigated and found unnecessary.

What did the dose-escalation study show about higher amounts?

That more ipamorelin put more of it in the blood in a straight line, but did not bring a matching rise in hormone. The study found that the PK parameters showed dose-proportionality1, meaning blood levels of the molecule rose in step with the amount infused.

The hormone side has a ceiling built in. The authors report that ipamorelin induces the release of GH at all dose levels with the concentration (SC50) required for half-maximal GH stimulation of 214 nmol/L1. Half-maximal means there is a top, and the paper also gives a maximal GH production rate of 694 mIU/L/h1. Beyond a certain concentration, additional molecule produces progressively less additional hormone.

People also differed. The inter-individual variability of the PD parameters was larger than that of the PK parameters.1 In plain words, the men cleared the molecule in much the same way but responded to it in different ways, so one amount gave different hormone surges in different men.

What does a figure in nmol/kg mean?

It counts molecules rather than weighing them, and it cannot be converted into micrograms without the molecule's mass. A nanomole is a fixed number of molecules, and expressing it per kilogram scales that number to body weight, which is how the 1999 human study and the pig experiments report their amounts.

The literature changes units from paper to paper. The 2012 rat study gave ipamorelin (0.014-0.14 µmol/kg)5, in micromoles, which are a thousand times larger than nanomoles, while the 2009 rat study and the 2014 human trial used milligrams per kilogram, which weigh the compound instead. The fish study specified a plain weight per fish with no adjustment for body weight.

Even a correct unit conversion would not turn an animal figure into a human one, since body size, clearance and route all differ, and this page converts none of them because a converted figure invites being used as a dose.

What amounts did the rat studies use?

Three rat studies report amounts. Each was set for a rat model of one specific problem, and none of them was about growth. In rats recovering from abdominal surgery, ipamorelin (0.01-1 mg/kg), growth hormone-releasing peptide (GHRP)-6 (20 microg/kg), or vehicle (saline) were administered via intravenous bolus infusion6, a bolus being a single quick injection.

In a second surgical model, rats got a range of amounts, and even the lowest, ipamorelin (0.014 µmol/kg intravenous) resulted in a significant acceleration5 of stomach emptying in those animals. The third study gave steroid-treated rats prednisolone and Ipamorelin (0.5 mg x kg(-1) x day(-1))7, prednisolone being a steroid that eats away at muscle.

Each of these was given to an animal whose gut or muscle had been harmed on purpose, and most went straight into a vein. They show what a rat was given in a lab, rather than a figure that carries across to a person.

What amounts did the pig and fish studies use?

The pig experiments report potency rather than a schedule. In conscious pigs, ipamorelin released GH with an ED50 = 2.3+/-0.03 nmol/kg8. ED50 is the amount that produces half of the maximum response, so it describes the compound's potency, rather than an amount anybody intended to administer repeatedly.

The same paper deliberately exceeded that amount to examine selectivity, reporting that the absence of a stress-hormone rise held even at doses more than 200-fold higher than the ED50 for GH release8. That is a test of how selectively the compound acts at high exposure, measured in pigs, rather than a safety margin anyone has confirmed in a person.

The fish experiment is the longest course on record, in which tilapia received either 5 or 30 µg of IPA for 21 days4, a fixed amount per fish rather than per kilogram. A three-week course in a small freshwater fish describes that particular experiment, and it says nothing about how long a person could use the compound.

Does it matter that every human amount went into a vein?

A great deal, because route decides how much of an amount reaches the blood. An infusion places all of it there directly. Any other route loses some along the way, and the share that arrives is called bioavailability, meaning the fraction of a given amount that reaches circulation.

The one non-vein route measured for ipamorelin shows how large that loss can be. In rats, after intranasal application, the bioavailability of ipamorelin was estimated at approximately 20%9, while related compounds reached about half. So the same amount sprayed into a rat's nose delivered roughly a fifth of what an infusion would.

Injection under the skin, the route most people use, has no measured figure for ipamorelin among these sources. A 2026 scoping review covering ipamorelin notes that significant heterogeneity existed in dosing and route of administration10 across this whole literature. An infusion amount carried over to a syringe under the skin is a different exposure, by an unmeasured margin, rather than the same dose given another way.

What strengths do ipamorelin vials come in?

The FDA recall record is the one public source that lists them, and it shows several. Recalled single-compound items include an Ipamorelin 3 mg Lyophilized 1 vial11, lyophilized meaning freeze-dried to a powder. Others were Ipamorelin, 9 mg injection12 and Ipamorelin, 15mg, injection13.

Some were supplied already dissolved. One recall covered Ipamorelin Acetate 9 mg/9mL Injectable vials14, and another an IPAMORELIN 2000MCG/ML INJ in 2 mL vial15. A liquid vial states a concentration, so its strength is fixed before anyone touches it.

Every one of these was withdrawn for lack of sterility assurance. They are packaging facts about batches that reached patients and were recalled, rather than a menu of strengths on sale now, and none of them implies an amount to use.

How do you reconstitute an ipamorelin vial?

The arithmetic is fixed and independent of any dose. A powder vial holds a set mass, and the volume of water added determines the concentration, so a 3 mg vial holds 3,000 micrograms whatever is added. Adding 1 mL of bacteriostatic water produces 3,000 micrograms per millilitre, and adding 2 mL produces 1,500 micrograms per millilitre.

A U-100 insulin syringe is marked in units, and 100 units make one millilitre, so each unit represents a hundredth of a millilitre; at 1,500 micrograms per millilitre, one unit contains 15 micrograms and ten units contain 150. Pre-dissolved vials eliminate the first step, since a 9 mg in 9 mL vial is already 1,000 micrograms per millilitre.

Arithmetic converts between mass, volume and syringe markings for any number a person brings to it, but it does not tell anyone which number to bring, because no study among the research behind this page established an amount for injection under the skin. Why is a study amount not a protocol?

Because each amount answered its own study's question, and none of those questions resembles what a person using ipamorelin wants to know: the 1999 rates were chosen to map blood concentrations, and the 2014 amount was chosen to accelerate bowel recovery, which it failed to do better than placebo.

Reviews of the field say as much. A 2026 review of peptides in ageing lists significant knowledge gaps include optimal dosing regimens16 for the compounds it covers, ipamorelin included. A 2026 critical review of peptides in sport adds that most published studies examine therapeutic applications under controlled dosing regimens17, which describes trials run under close supervision rather than use at home.

A protocol would need a goal, a group of people, a route and proof that the amount does some good at a cost worth paying. For ipamorelin the record gives two narrow groups, one route and a missed goal. That is a set of study settings, rather than a dose anyone has shown to work.

What we don’t know

The gaps in the evidence matter as much as the findings.

  1. 01Any amount for a healthy person using it outside a study. The only repeated human schedule was set for patients recovering from surgery.
  2. 02What happens when it is injected under the skin. How much reaches the blood by that route has not been measured in any species among these sources.
  3. 03Whether the growth hormone response fades with repeated use. The volunteer study gave one infusion per person.
  4. 04Whether women respond differently. The healthy-volunteer study enrolled men only.
  5. 05What a course longer than seven days does. No human study among the research behind this page ran longer.
  6. 06Whether cycling, breaks or tapering change anything. None has been tested.
  7. 07How animal amounts relate to a person. The rat, pig and fish figures were set for those animals, and no study among these sources bridges them.
  8. 08What a vial on sale contains. The recall records concern withdrawn batches and do not describe current stock.
